An unannounced Required – 1 Year Inspection was conducted on 03/09/22 at 9:50 AM by Licensing Program Analyst (LPA) Katrina Chicote. LPA conducted COVID-19 screening call prior to entering facility. Facility is currently licensed for a capacity of 90 and has 32 children enrolled. Licensing staff met with Assistant Director, Edwina Rambo (Brown). LPA was taken on a guided tour of the facility of both indoors and outdoors. Director, Micha Mims, arrived at 10:00 AM and concluded visit with LPA.

At the initial start of the site visit, there is a total census of 20 children and a total census of six teachers present. The facility is made up of six classrooms but facility is only using four of six classrooms at time of inspection. Facility is currently using Room 1, Room 4, Room 5, and Room 6. Telephone service, heating, lighting and ventilation were evaluated. At 10:04 AM, LPA began touring classrooms. Furniture and equipment were inspected for age appropriateness and good repair in all classrooms. LPA observed child size tables and chairs, dramatic play toys, reading area, plastic manipulative toys, art materials, and other age appropriate toys available for children free of loose and sharp parts at time of inspection. LPA observed rooms of each classroom to have linoleum floors and carpets and to be free of hazards at time of inspection. There were cubbies observed as storage for children's belongings in each classroom. LPA observed Mats/cots stored in bathrooms of each classroom and were observed to have child's names labeled with no linens. Per Assistant Director, new linens and blankets are provided by the facility daily. Availability of drinking water was reviewed. LPA observed water pitcher, water bottles, and children's personal water bottles being stored in each classroom. LPA observed each classroom to have it's own bathroom and one toilet and one sink. Age appropriate sinks and toilets were inspected for availability, good repair, water temperature, toilet paper, area safety and sanitation. At 10:20 AM, LPA finished tour of classrooms.

LPA observed a first aid kit is kept in each classroom hanging on wall by classroom sink. Carbon monoxide detectors and smoke detectors are present in the facility. Fire extinguishers have been serviced in 08/09/21. Hazardous items including poisonous cleaning compounds were stored inaccessible to children. LPA observed these items to be stored in off limits kitchen located in facility's multi-purpose room that was not being used at time of inspection.

At 10:22 AM outdoor equipment was inspected for safety, cushioning material, good repair and age-appropriateness. LPA observed Room 5 to be using outdoor space at time of inspection. Per Assistant Director, facility staggers outdoor playtime between classrooms. LPA observed a large play structure in the middle of outdoor playground that is well maintained that has three slides, plastic rock wall for climbing, bridge, and tunnels. LPA observed cushioning material under structure to absorb fall to be well maintained at time of inspection. Required shade and fencing were inspected. LPA observed a shaded area with tables next to multi-purpose room that also had a wooden play kitchen that is well maintained available for children in care. LPA observed child size bicycles in use by children around bicycle path to be well maintained and handle bars in tact. LPA Chicote advised Assistant Director that the children need to be within the direct care and supervision, including visual supervision of the teacher(s) at all times. The children have access to drinking water during outside play through personal water bottles. Play area was inspected for hazards and inaccessibility to bodies of water; no bodies of water or hazards observed

Snack/lunch menus were reviewed. Food and snacks were reviewed for availability, quantity and appropriateness to children in care. At 10:24 AM, LPA inspected food preparation areas for safety, cleanliness and proper equipment. Facility serves AM Snack, Lunch, and PM Snack.

Teacher-child ratios were observed and staff names recorded. Care and supervision was evaluated to determine if the basic needs of children are met and appropriate. Sign-in and out sheets and procedures were reviewed. LPA observed all 20 individual sign in sheets following Title 22 regulation requirements. Personal Rights of children were observed by LPA.

At 11:00 AM, LPA reviewed Staff and Children’s Records. Criminal Record Clearances were reviewed for adults. CPR card expires on 05/2023 for Assistant Director. Inspection of required forms made. Immunization Records were not available for 3 out of 7 staff at the facility.

Incidental Medical Services:
This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. The following were discussed with the Director:
The Director was advised that inaccessibility of hazards must be constantly reassessed depending on the children in care.

The fire extinguisher type 2A-10BC must be serviced annually or as often as necessary. Smoke and Carbon Monoxide detectors should be checked and batteries replaced as needed. Changes should be reported to the Department as soon as they occur such as construction, remodeling, telephone number changes and/or if you move from your location.


Mandatory Forms for the childr
en’s files and staff files, requirements for fire drills, earthquake drills and documentation were discussed. Role and responsibilities of being a Mandated Reporter were reviewed. The Director was advised how to access forms and Regulations online at www.ccld.ca.gov. Director was made aware that it is his/her responsibility to know the regulations as well as anyone who assists in providing care.

Licensee (or facility representative) was reminded that all adults 18 and over working in the facility, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Senate Bill 792: this bill, commencing September 1, 2016, prohibits a person from being employed or volunteering at a child care facility or family day care if he or she has not been immunized against influenza, pertussis and measles. LPA discussed the influenza waiver during the visit.

Based on this information, the following deficiencies on the attached LIC 809D are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children’s health & safety.



The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site inspection by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
