13 | Licensing Program Analysts (LPA) Cynthia Reyes, arrived unannounced at the facility for the purpose of conducting a follow up complaint Investigation regarding the allegations listed above. LPA met with Melissa Wojcik, Administrator/Interim Director who took LPA on a tour of the facility.
Based on LPA Interviews conducted and documents reviewed and received, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED, regarding the facility vehicles are in disrepair, Staff failed to supervise day care children resulting in them wandering off, Staff failed to pick up daycare children in a timely manner. California Code of Regulations, Title 22, Division 12, Chapter 3, are being cited on the attached LIC 9099D.
Interviews and LPA own observation determined that the facility vehicles are in disrepair. One van would turn off at a red light/middle of the street, seat belts were not locking properly, seats in one of the van are not bolted down and when the van would stop the seat would move forward with a child in it, fabric on van ceilings are torn, vans are stained and dirty inside. |
