32 | Allegation: Staff do not ensure children’s diapering needs are met
According to the allegation, staff do not change children’s diapers timely during the morning hours. During interviews, Staff #1 stated children’s diapers were checked every 30 minutes and “we write down if they are wet, BM (bowel movement), or dry and the time and your initials”. Staff #2 also stated they write in the log if the child’s diaper is dry when checked. When asked what time the children start arriving at the facility, Staff #2 stated, “Lately they have been getting here about 6:40 am. Usually it is a lot of Care Bears (2-year-olds)”. LPA obtained the facility diaper log for dates 6/13/23-11/8/23. Of the 106 diaper log entries reviewed by LPA, the first entry is at or after 8:00 am in 96 entries. When asked why most of the first entries are not until after 8:00 am, Staff #1 stated “…they aren’t full diapers enough to be changed until that time”. Staff #1 stated if there is some urine in the diaper, they don’t change them until it is full. When asked if staff have ever waited to change a diaper that needed to be changed, Staff #2 stated that on 10/9/23, they “misplaced a key for the Care Bears room, so we had to wait to change diapers”. LPA observed the diaper log for the date of 10/9/23 had the following first entries: 9:00 am (BM); 9:05 am (Peed Through); 9:05 am (Wet); 9:05 am (Wet); 9:10 am (BM Red). During interviews, Parent #9 stated their child. “was sent home not wiped well. They had crumbs inside the diaper and poop was caked on the inside of their cheeks”. Parent #9 also stated on another occasion staff told Parent #9 that they couldn’t get in the classroom cuz the door was locked and they told me my child didn’t get changed right away”.
Based on observations, records reviewed, and interviews conducted by LPA, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. California Code of Regulations, (Title 22, Division 12 & chapter 1), are being cited on the attached LIC 9099D.
