An unannounced Case Managment-Deficiencies Inspection was conducted on this date by Licensing Program Analysts (LPA) Cynthia Reyes. Upon arrival LPA observed the teacher child ratio was not met.

The following is being cited in accordance to Title 22 of the California Code of Regulations:
LPA observed 14 preschool children alone with 1 teacher Sakunthala Goonetilleke. LPA walked outside to the outdoor play ground as there were no staff present inside the facility. As LPA walked into the small enclosed play yard, Nilani Leula, Director was walking out with one child and stated she was taking him inside to his father. LPA asked how many children were present and she stated 14 and walked inside the facility. LPA remained out side and counted the children with Teacher Ms. Goonetilleke. 14 children were counted as well as the one child who was walked inside to his father. The Teacher was left outside with 14 children.

LPA did not observe the notice of site visit posted on the parent board located at the front entrance of the facility where the parents sign in/out and all the other parent documents are posted. Director showed the LPA and stated she posted the Notice of site visit in the hall way between the class rooms and the kitchen because the parent board was full and the parents walk threw the hall way. LPA observed that none of the type A or report was posted with the notice of site visit. Director stated she did not know all the report was suppose to be posted and the notice of site visit is the only form that says it has to be posted. LPA also reviewed the pile of LIC 9224 Acknowledgement of Receipt of Licensing Reports the director has that she stated she had not put them in the child file yet after the parent signed them. LPA observed each form had something wrong with it such as dates, child first name only, wrong boxes marked off, or just blank information. LPA asked for copies of the Acknowledgement of Receipt and Director changed dates on forms and then gave the LPA copies. LPA had Director initial each area of what she changed on the documents. LPA made a telephone call to LPM Guangorena for guidance and to inform of all the issues with the director during the inspection.

Upon receipt, the Licensee shall post the Notice of Site Visit and the Licensing report.

This report and the Notice of Site Visit shall be posted for thirty (30) consecutive days.

Failure to maintain posting as required will result in the issuance of a citation and the assessment of a $100 civil penalty.

A copy of this report shall be provided to the parents/guardians of the children currently enrolled by the next business day or immediately upon return.

A copy of this report shall also be provided to the parents/guardians of any children newly enrolled at the facility for the next twelve (12) months.

The LIC 9224 Acknowledgement of Receipt of Licensing Reports must be maintained in each child's file immediately upon receipt from the parent.

LPA provided Licensee with a blank copy of the LIC 9224 Acknowledgement of Receipt of Licensing Report.

An exit interview was conducted with Nilani Leula, Director. A copy of this report has been signed and given and appeal rights were provided and explained.
