On October 25, 2024, at 1:50pm Licensing Program Analyst (LPA) A. Carter conducted an Unannounced Required Annual inspection and met with licensee Mariana Sinohui. Also present was licensee’s husband, son, son's fiance, and assistant. LPA disclosed the purpose of the inspection and was granted entry into the facility. Licensee was provided a copy of the Facility Entrance Checklist.

All adults associated to the home were found to have criminal record clearance. There were zero (0) daycare children present during today’s inspection. Licensee states that there are currently seven (7) children enrolled. The children's roster was reviewed and is current. Licensee reports that the facility’s hours of operation are Monday thru Friday from 6:00am to 4:30pm. Licensee does provide transportation but does not provide overnight care.
The Parent Board had the following required documents posted:

v Earthquake Preparedness
v Emergency Disaster Plan
The Disaster Drill log was current with the last drill conducted on September 12, 2024. All life saving devices such as Carbon Monoxide Detector, Smoke Detector, and Fire Extinguisher were observed by LPA. Dual Carbon Monoxide and Smoke Detectors were tested and operable at the time of inspection. The 2A10BC Fire extinguisher was fully charged with last service date of January 2, 2024.
· At 2:15pm school age child came to the home after school. Parent picked child up at 2:30pm.

This is a story home which consists of three (3) bedrooms and two (2) bathrooms, kitchen, living room, garage, front and backyard. Areas used by the children include the living room, kitchen, one (1) bathroom located in the hallway. Per Licensee areas off-limits to children and parents include three (3) bedrooms, and one (1) bathroom, garage, and backyard. Per licensee, off limit areas are also locked during operating hours, LPA advised licensee to use safety knobs to assist with making off-limit areas inaccessible to children in care.

All areas identified on the facility sketch as accessible to children were inspected to ensure that they are clean and orderly with ventilation and heating for the safety of the children. Licensee states there is central air and heating throughout the home. There is a working telephone maintained in the home via cellphone. The isolation area for sick children waiting to be picked is in the living room, supervised away from the other children in care.

The living room is used as the main care area. LPA observed age appropriate toys available such as books for reading, cars, and puzzles free of loose and sharp parts. Per licensee children will have access to bathroom located in the hallway. LPA observed the bathroom to be clean with an operable sink, faucet, and toilet. LPA inspected cabinets in the restroom where hygiene items are stored inside.
· At 2:45pm LPA advised licensee that medication should be inaccessible to children in care. The medications are located in the upper kitchen cabinet but are accessible to the older children. Licensee removed medicine to off-limit area making it inaccessible to all children in care during inspection.
Per licensee, food is provided for children in care. LPA observed kitchen to be free of hazards and refrigerator was observed to be in good condition at time of inspection. Knives are kept in upper kitchen cabinets, cleaning supplies are kept in an upper cabinet in the hallway, detergents are kept in the off limits garage. The Licensee states that there are no poisons in the home The Licensee does understand that poison must be locked. The Licensee was advised that inaccessibility of hazards must be constantly reassessed depending on the children in care.

Per licensee, the children will have access to the front yard for outside play. Per Licensee, children are supervised while playing outside. LPA observed yard has grass and adequate perimeter fencing through-out the property. Play area is free of loose and sharp articles or objects and all trees, shrubs, and plants are maintained.

Infant Care: Currently licensee has 1 infant enrolled. Per Licensee, there are no infants under 12 months enrolled. LPA observed a cots used for napping. Licensee was advised Infants shall not be swaddled in care. Car seats shall not be used for sleeping. LPAs discussed the new Safe sleep regulation 15-minute sleep check documentation for infants 0-24 months.

Children’s records were reviewed for (LIC) 282- Affidavit Regarding Liability Insurance, Immunization's Records, LIC 700- Identification and Emergency Information, LIC 627- Consent for Medical Treatment, LIC 995A Notification of Parents’ Rights, and documentation of 15-minute Infant Sleep Check (0-24 months).

Licensee / staff records were reviewed: Licensee Pediatric First Aid/CPR expires on 6/2025. Staff had proof of required immunizations.

· Proof of TB clearance for license and assistant required.

· Licensee Mandated Reporter training expired 9/15/22. Updated Mandated Reporter certificates for licensee and assistant required.

· Pediatric first aid / CPR certificates for licensee and assistant required.

Licensee states there are weapons in the home. Firearms are stored separately from ammunition in separate locked containers.

Licensee stated they do not have any children that needs medication administer, or any children with allergies in care at this time.

Licensee stated that there are no smokers in the home.

Licensee has five (5) small dogs in the home. LPA observed pets at time of inspection.

Licensee stated there are no bodies of water around the premises. LPA did not observe any bodies of water at time of inspection.

Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee [or facility representative] of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. The following information regarding ADA was provided: US Department of Justice (USDOJ) toll-free ADA Information Line at (800) 514-0301 (voice)/ (800) 514-0383 (TTY) and link to publication: Commonly Asked Questions about Child Care Centers and the ADA, available at: https://www.ada.gov/resources/child-care-centers/.

Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

During the exit interview, the LICENSEE Mariana Sinohui, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS.

Based on this information, the following deficiencies on the attached LIC 809D are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children’s health & safety.

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site inspection by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
