Licensing Program Analyst (LPA) Roxana Lopez conducted an unannounced annual random inspection to the above facility. LPA met with Assistant, Stephany Ramirez, to whom the reason for the visit was explained and gave LPA a tour of the facility. Per Assistant, Licensee was out doing errands. There are 16 children currently enrolled. There were 4 children present upon arrival- also present was Assistant Teresa Palacios. The facility was observed to be operating within the license capacity limitations.

The Asssitant states that 4 adults and no children currently live in the home. All adults living in the home have obtained a criminal record clearance.

This is a one-story home which consists of 3 bedrooms, 1 Playroom, 2 bathroom, living room, dining room, kitchen, laundry room, front yard, detached garage with guest room and guest bathroom, backyard patio which is fenced.

Per Assistant, areas off limits to children and parents include 2 bedrooms, guest room, guest bathroom and front yard. The children have access to kitchen, dining room, living room, 1 bathroom, 1 bedroom, playroom with bathroom and backyard patio which is fenced. LPA advised Assistant that eating, and sleeping is not permitted in the playroom.

Facility License, Emergency Disaster Plan, Parent’s Rights Poster, and Earthquake Preparedness Checklist were observed to be posted in the playroom of the home. A current children’s roster was available for review.

There is telephone service via landline at the facility. Hours of operation are Monday – Friday, 6:00am-6:00pm. There is ventilation and central heating in the home. Per Assistant all meals are provided for children in care. Licensee transports school age children to school.

Detergents, cleaning compounds, and medications were observed to be inaccessible to children. LPA advised that all poisons should be locked under key or combination lock. Assistant states that there are no firearms or weapons stored in the home.

The valve on the required 2A 10BC fire extinguisher indicates fully charged and was serviced on 8/22/2024, as indicated on service tag. Smoke detectors were observed to be operable.

At 10:00 am LPA observed that there is no carbon monoxide in the facility. Per Assistant, they believed smoke detectors were combined. LPA obtained pictures- which showed detectors were only smoke detectors.

All homes shall conduct fire and disaster drills at least once every six months and document the date and time of each drill. Last drill documented was conducted on 1/8/2025. The home is observed to be clean and orderly. There are first aid supplies available. There are age-appropriate toys available for children.

as an additional resource. LPA also informed facility representative of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

LPA did not observe the following items during the inspection: Infant Walkers, Johnny Jumpers, Saucer Chairs, Trampolines and/or any other item that fall into these categories are not permitted in a family child care facility.

Smoking is prohibited in a licensed Family Child Care Home. Per Assistant , no one smokes in the home.

Currently, children are using the back Patio for outdoor play time. The outdoor play area was observed to be fenced. LPA observed that the outdoor yard has toys and other materials for children to play with. No bodies of water were observed on this day. There is 1 dog on the premises. Per Assistant, the dog is kept in off limits areas during daycare hours.

At 10:30 am children’s records and staff records were reviewed. LPA observed that Child # 3 and Child # 4 were missing form LIC 995 A parents rights- Child # 4 was missing form LIC 700 Identification and Emergency information. Both Licensee and Staff # 2 were missing proof of required immunizations.

The licensee and other personnel have completed training on preventive health practices including Pediatric First Aid and CPR. The licensee's Pediatric First Aid and CPR expires on 4/2025. The Mandated Reporter AB 1207 compliant Child Care Training Certificate on file. LPA issued a copy of the LIC 857 Children’s Records Review and the LIC859 Staff Records Review to the licensee during this inspection. Page 3 of 5

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. Facility representative was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.



LPA advised the licensee to access forms, regulations and quarterly updates on line at: www.ccld.ca.gov.

Facility representative was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

During the exit interview, the Assistant, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS.

Based on the LPA’s observations and records reviews, the following deficiencies listed on the attached LIC 809D (deficiency page) are being cited in accordance with California Code od Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children’s health and safety.

A notice of site visit was given and must remain posted for 30 days.
