Licensing Program Analyst (LPA) Rita Ramos conducted an unannounced required 1 year inspection to the above facility on 10/21/2021. LPA arrived at the facility at 12:25PM, identified self and met with Lisa Fielding, Head Teacher, who guided analyst on a tour of the facility. LPA provided Head Teacher with a copy of the LIC 125 Entrance Checklist to help facilitate the inspection. This is a preschool program which consists of 4 classrooms; Room 80, 81, 82, and 83. Facility operation hours are Monday to Friday from 7:45AM to 2:45PM for the full day sessions in Rooms 81, 82, and 83. Room 80 has 2 part day sessions from 7:45AM to 11:15AM for the AM session and 12:15PM to 3:45 for the PM session. LPA was later met by Program Administrator of Staff Services, Jenny Acosta, and Early Learning Center Manager, Patsy Le Blanc, during the inspection.

All areas identified on the Facility Sketch were inspected. Upon arrival, the following staff were present during this inspection: Room 80 Staff #3, #4, #5, and #6 with 9 preschoolers; Room 81: Staff #1 and #2 with 5 preschoolers; Room 82: Staff #8 and #9 with 9 preschoolers, and Room 82: Staff #7 with 3 preschoolers. The facility was observed to be within the license capacity and limitations. The following was observed during the tour of the facility:

Furniture and equipment were inspected for age appropriateness and good repair. LPA advised that Teacher and Program Administrator to ensure that the telephone is always operating in case of an emergency due to LPA calling the facility prior to and at arrival time and receiving no answer. Children have their own cubby to store their belongings. Linens are washed weekly by a laundry service. Napping equipment (cots) were observed in the classrooms. Per Head Teacher, the isolation area is located in the nurse's office. Age appropriate sinks and toilets were inspected for availability and good repair in all restrooms. General sanitation was observed. Availability of indoor drinking water was observed in classrooms. Disinfectants, cleaning solutions, medication and other items that are dangerous to children, were inaccessible to children. ---Page 1 of 3

Per Program Administrator there are no poisons stored at the facility. Carbon monoxide detectors were observed and are operable.

There is no kitchen at the facility. All meals and snacks are delivered from the nearby elementary school cafeteria. Trash cans used to discard food have tight fitting lids.

Outdoor playground equipment is in a safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All areas around or under high climbing equipment, slides, and similar equipment are cushioned with material that absorbs a fall. There is adequate shade in the play yard. Availability of outdoor drinking water was observed. LPA advised that no children shall be left without the supervision of a teacher at any time.

All floors were observed to be clean and safe. All materials accessible to children were observed to be toxic-free There are no firearms stored on the premises. There are no pools or bodies of water at the facility.

All staff and employees have obtained a criminal record clearance or criminal record exemption as a condition of employment with the Long Beach Unified School District. There is at least one person trained in CPR and Pediatric First Aid present during this inspection.

Children’s Records were reviewed and documented on the LIC 857. Children's roster was reviewed and is current. Sign-In and Sign-Out sheets were reviewed. Children present were signed in. Disaster drill log was available. Per Teacher, facility participated in the Great CA Shake-Out.

LPA also reviewed staff records. The review of Staff records was documented on the LIC 859. Staff present have proof of the AB 1207 Mandated Reporter Training certificate on file except for Staff #3 who has a district mandated reporter training. LPA advised that Staff #3 take the AB 1207 training and submit a copy by 11/11/21. All staff have been given on the-job training sanitation principles, housekeeping, including universal health precautions. LPA did not review confidential forms and information due to the files being stored at the administrative office.

First Aid supplies were observed in the classroom. According to the Head Teacher, there are currently no children on medication.

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department.
