Licensing Program Analyst (LPA) Judy Mora conducted an unannounced initial 10 day inspection to investigate the above complaint allegation. LPA toured facility with Director, Michelle Johnson at approximately 11:35 AM. LPA observed 01 staff with 2 infants in the napping room and 05 infants with 02 staff in the indoor activity space.

During the course of this inspection LPA visually observed the infants and staff in the classroom to determine if needs of children and ratios are being met. LPA inspected the facililty both indoors and outdoors. LPA also obtained a copy of the facility roster. Based on the available information, the preponderance of evidence standard has been met, therefore the above allegation is found to be Substantiated. California Code of Regulations,(Title 22, Division 12 & Chapter Number 6), are being cited on the attached LIC. 9099D. LPA observed 3 stacked crates on the outdoor playground as well as a cooking pan set on top. This s poses a potential Health & Safety risk to clients in care.

Upon receipt of this report, the licensee shall post ANY licensing report documenting a type “A” citation or substantiated complaint. This must remain posted for 30 days during hours of operation. In addition to posting this report, the licensee will also provide copies to the parents of the children in care for up to one year.

A copy of the LIC 9224 - Acknowledgement of Receipt of Licensing Reports was provided.

Exit interview was conducted with Director. Appeal rights and procedures were explained.
