13 | Licensing Program Analyst (LPA) Judy Mora conducted an unannounced initial 10 day inspection to investigate the above complaint allegation. LPA toured facility with Director, Michelle Johnson at approximately 11:35 AM. LPA observed 01 staff with 2 infants in the napping room and 05 infants with 02 staff in the indoor activity space.
During the course of this inspection LPA visually observed the infants and staff in the classroom to determine if needs of children and ratios are being met. LPA inspected the facililty both indoors and outdoors. LPA also obtained a copy of the facility roster. Based on the available information, the preponderance of evidence standard has been met, therefore the above allegation is found to be Substantiated. California Code of Regulations,(Title 22, Division 12 & Chapter Number 6), are being cited on the attached LIC. 9099D. LPA observed 3 stacked crates on the outdoor playground as well as a cooking pan set on top. This s poses a potential Health & Safety risk to clients in care.
