On 5/20/22 LPA conducted an unannounced case management inspection at the above facility. LPA met with the Licensee Silvia Preciado Camarena. There were 3 children present and 3 adults. LPA reviewed the Facility associations list and Adult 1 was not observed on the list. LPA checked the criminal record clearance of the individual with the OD at the regional office. It was then confirmed the individual does not have a complete criminal record clearance. Adult 1 left the facility during the inspection.
During the inspection LPA also observed a baby walker located in the day care area. LPA did not observe any child using the walker. The walker was removed from the day care area.

The deficiencies listed on the following page were observed by the LPA and are being cited in accordance with California Code of Regulations Title 22. Please see attached LIC 809-D. Deficiencies that are being cited need to be cleared to protect the children’s health & safety.
The notice of site visit was posted where the parent/guardian of children enter and exit the facility. A copy of this report shall also be posted where the parent/guardian of children enter and exit the facility. Both the notice of site visit and licensing report shall remain posted for 30 consecutive days. Failure to maintain posting as required will result in a $100.00 civil penalty. A copy of this report shall be provided to the parent/guardian of children currently enrolled by the next business day or immediately upon their return. A copy of this report shall also be provided to the parent/guardian of any newly enrolled child for the next 12 months. A signed Acknowledgement of Receipt (LIC9224) shall be in each child’s file, acknowledging receipt.

A notice of site visit was given and must remain posted for 30 days.
