Licensing Program Analysts (LPAs) Rita Ramos and Thelma Razo conducted an unannounced annual inspection on 07/22/2021 at 9:00AM. LPAs met with Amber Johnson, Teacher, who guided analyst on a tour of the facility. This is a preschool program which consists of 3 classrooms; Main Room, Circle Time Room, and Reading Room. Facility operation hours are Monday to Friday from 6:00 AM to 6:00 PM.

All areas identified were inspected. Upon arrival, the following staff were present during this inspection: Main Room: Staff #1 and #2 with 10 preschoolers. The facility was observed to be within the license capacity and limitations. The following was observed during the tour of the facility:

While inspecting the furniture and equipment, at 9:09AM LPAs observed that there is a torn child-size sofa in the Reading Room. LPAs also observed at 9:09AM that there is paint chipping off from the ceiling portion between Main Room and Reading Room (pictures were taken). Children have their own cubby to store their belongings. Linens are washed weekly at the facility. Napping equipment (cots) were observed to be stored in the Circle Time Room and in the Reading Room. Age appropriate sinks and toilets were inspected for availability and good repair in all restrooms. However, LPAs did smell a strong scent of urine. LPAs advised the Teacher to open the restroom window for ventilation or clean and disinfect the restroom frequently throughout the day. Availability of indoor drinking water was observed via a faucet.

Teacher states there are no poisons stored at the facility. Carbon monoxide detectors were observed and are operable.

All kitchen areas/food preparation areas and food storage areas are kept clean and are free of litter, rubbish, rodents, and/or any other vermin. ----Page 1 of 3

All storage containers for solid waste, including moveable bins have tight-fitting covers that are kept on and in good repair. Trash cans used to discard food have lids that are kept on.

When inspecting the outdoor playground equipment, at 9:14AM LPAs observed that the drinking faucet was covered in dirt and not functioning (pictures were taken). At 9:15AM LPAs observed dirt and spider webs inside play structures and play tunnels. At the same time LPAs observed that one of the play structures is a play house and inside the play house there is a toy sink and cabinet that were covered with dirt and broken (pictures were taken), another play house made of wood has splinters exposed, and there is a broken gazebo. LPAs advised Teacher that the play house should be discarded due to the broken areas exposing sharp edges, in addition, LPAs observed a large spider (pictures were taken). When inspecting the surface of the outdoor activity space, at 9:19AM. LPA observed that the rubber matting under the slide is worn down and in another area a piece is missing and can cause a tripping hazard (pictures were taken). There is adequate shade in the play yard. Availability of outdoor drinking water was not observed. At 9: 54AM LPAs observed children going outside to play in the outdoor area and at 10:01AM LPAs observed Child #10 walk into the facility from being outdoors to drink water indoors and walk back out to go outside. LPAs advised that no children shall be left without the supervision of a teacher at any time and that water should be readily available both indoors and outdoors.

All floors were observed to be clean and safe. All materials accessible to children were observed to be toxic-free There are no firearms stored on the premises. There are no pools or bodies of water at the facility.

All individuals present have obtained a criminal record clearance or criminal record exemption. There is at least one person trained in CPR and Pediatric First Aid present during this inspection.

Children’s Records were reviewed. In reviewing children's records, LPAs observed that Child #2, #4, #6, and #7 had incomplete medical and emergency information.

LPAs also reviewed staff records. The review of Staff records was documented on the LIC 859. Staff present did have proof of the AB 1207 Mandated Reporter Training certificate on file. Staff present did have proof against TB, measles, pertussis, and influenza. All staff have been given on the-job training sanitation principles, housekeeping, including universal health precautions. -----Page 2 of 4

Children's roster was reviewed and is current. Sign-In and Sign-Out sheets were reviewed, however, in review of sign-in and out sheets at 10:36AM, LPAs observed that Child #1, #2 and #6 were not signed-in. Menus are posted one month in advance. LPAs advised that the menu should be posted where it is visible by the child's authorized representative.

When inspecting the children's files, LPAs observed that Child #10 has an LIC 9168 Nebulizer Care Consent form on file. Per Teacher, the facility does not administer medication. When LPAs asked about the nebulizer form for Child #10, Teacher informed LPAs at 10:27AM that child has a nebulizer that parent administers but it is stored in child's cubby. LPAs discussed medication storage and advised Teacher to store medication in a locked container and to make it inaccessible to children in care. The facility was observed to be equipped with an isolation area for any child who becomes ill and it is located in the office or it can also be the reading room.

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. LPAs advised the licensee to access forms, regulations and quarterly updates on the Child Care Licensing website at: www.ccld.ca.gov. LPA provided the Teacher with a copy of the Effects of Lead Exposure form during the inspection and discussed the following Provider Information Notices (PINs) Required Lead Testing: PIN 20-01 CCP Effects of Lead Exposure, and Guardian User Account Access - PIN 20-20-CCLD.



The following deficiencies listed on the attached deficiencies page are being cited in accordance with California Code of Regulations Title 22.

A copy of this report shall be provided to the parent/guardian of children currently enrolled by the next business day or immediately upon return. A copy of this report shall also be provided to the parent/guardian of any newly enrolled children for the next 12 months (1 year). The Acknowledgement form must be maintained in each child’s file immediately upon receipt from parent. Teacher was provided with a copy of the parent Acknowledgement of Receipt of Licensing Reports Form during this visit. A copy of the Parent Notification Requirements was also provided to the licensee/Director.

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. Exit interview was conducted with Amber Johnson, Teacher, including, but not limited to Provider Rights, Appeal Procedures and Agencies Consultative Role.
