13 | Licensing Program Analyst (LPA) Susann Sanchez conducted an unannounced complaint inspection on 08/10/21. LPA arrived at the facility at 1:30pm. LPA met with Yolanda Duckett, Licensee, for the purpose of delivering the findings for the above allegation. Licensee gave LPA a tour of the facility at 1:38pm. LPA observed, 6 children, 3 school age and 3 staff members present during the inspection.
During the investigation, interviews were conducted with Licensee, 2 staff, 6 parents, and 2 children. None of the interviews conducted corroborated the allegation of Licensee is giving day care children medication without consent.
Although the Licensee has given medication in the past, the medication was confirmed to be taken as directed. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation(s) did or did not occur, therefore the allegation is unsubstantiated.
The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit made by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. Exit interview was conducted with Yolanda Duckett, Licensee, including, but not limited to Appeal Procedures, Site Visit and Initial Appeal Rights.
