Licensing Program Analysts (LPAs) Ariel Cazares and Alanna Gontarek conducted an unannounced 3-Year Required inspection on this date. LPA met with Director Melina Petrosyan sand was guided on a tour of the facility. There were 24 children present and two staff. The facility’s hours of operation are Monday through Friday, 7:00am-6:00pm.

LPA toured the areas according to the facility sketch on file. Per Director, no alterations have been made and LPA observed none. The facility consists of 3 classrooms. Each classroom was inspected. Furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting and ventilation were evaluated. Children have their own space in each classroom to store their belongings. Napping equipment is available in form of collapsible cots. Reminder: Linens are to be taken home each week to be washed. The facility is in compliance with this. There is available drinking water in form of personal water bottles that can be refilled. There are first aid and emergency supplies located in the classroom in a cabinet. The children’s restrooms are located in classrooms . The restrooms were observed to be clean and in good condition, with sufficient supplies for children to use.

Per director, the isolation area is located in the office and a staff restroom is available for ill children to use. The office is used to store medication for children. There is a kitchen located on the second floor of the facility, where snacks and lunch is prepared. A hot lunch is offered as an option, but children bring their lunch from home. LPA test the carbon monoxide detector located near the kitchen and found it to be operational.

LPA inspected the children’s outdoor space. The playground was inspected for safety, cushioning material, good repair and age appropriateness. LPA observed there to be shade and adequate fencing. Personal water bottles are brought outside by the children for drinking water.

Teacher child ratios were observed to be in compliance. Care and supervision was evaluated to determine if the basic needs of children are met.

LPA also reviewed criminal record clearances, children and staff records. LPA found the records to be complete and in order. Sign in and out sheets were reviewed and found to be accurate.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. No deficiencies were observed or cited on this inspection.

The Notice of Site Visit (LIC 9213)must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
