An unannounced Required Inspection was conducted on this day by Licensing Program Analyst (LPA) Seung Lee. Facility is currently licensed for a capacity of 35 children. LPA met with May Sabbah, Site Director, who guided analyst on a tour of the facility at 11AM. This is a Head Start preschool program which is operated by Pacific Asian Consortium in Employment (PACE) and is located on the premises of Casa Shalam. The program consists of three sessions in 2 classrooms. Days & Hours of Operation are from: M-F; 1 morning session is from 7:00AM -01:30PM, the other morning session is from 8AM-2:30PM. The afternoon session is from 2PM-5:30PM.

All areas identified on the Indoors and Outdoors Facility Sketch were inspected. LPA observed a total of 14 children with 2 Staff members in Room 02 and 14 children with 3 Staff members in Room 01. Teacher child ratios were observed and staff names recorded. Sign in and out sheets and procedures were reviewed.

INDOORS: Furniture and equipment was inspected for age appropriateness, good repair and free of sharp, loose, or pointed parts. Telephone service, heating, lighting and ventilation were evaluated. There is no napping taking place since the program only offers part time day sessions. Storage for children's belongings and isolation area were inspected. Ill isolation area is in the office reception area or in the quiet area of each room; the children will use the staff restroom if needed. Drinking water is readily available indoors for children to drink freely via a water jug. Carbon monoxide detectors and smoke detectors are present in the hall way. Age appropriate sinks (4) and toilets (3) were inspected for availability, good repair, water temperature, toilet paper, area safety and sanitation. First Aid supplies were inventoried. A review of the medication policy, including administering, labeling, storage, and records was made.

Food menus were reviewed. Food is provided by the Central Kitchen. Food and snacks were reviewed for availability, quantity and appropriateness for children in care. The First sessions with the 2 classrooms receives breakfast , lunch and a snack, the Second session in the afternoon with 1 classroom receives PM snack and supper.

Food preparation area was toured for safety, cleanliness and proper equipment. All storage containers for solid waste, including moveable bins had tight-fitting covers that are kept on, and in good repair. The facility was observed to be free of flies, other insects and rodents. Disinfectants, cleaning solutions, medications and other items that are dangerous to children, were inaccessible to children

OUTDOORS: This facility has a waiver for staggered outdoor play schedule, with an approved max capacity of 18 children at one time. The outdoor play yard is completely fenced and exclusively used by the Head Start program. Outdoor play equipment was inspected for safety, free of sharp, loose pointed parts, good repair and age appropriateness. Required shade, drinking water and fencing were inspected. Drinking water was available outdoors for children to drink freely via a water jug and two water fountains. Play area was inspected for hazards and inaccessibility to bodies of water; no bodies of water or hazards were observed. All areas around or under high climbing equipment, swings, slides, and similar equipment are cushioned with material that absorbs a fall.

RECORDS REVIEW: At 11:30AM Children's records were reviewed. Sampling of children’s emergency information was reviewed and appeared to be current. Staff files were also reviewed

Incidental Medical Services (IMS). The facility provides Incidental Medical Services-IMS. LPA reviewed storage of medication and (equipment/supplies, and reviewed children’s, personnel, and administrative records. (LIC. FACILITY IS PROVIDING IMS. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. A Plan of Operation that includes IMS must be submitted to the Department. No deficiencies were cited during this inspection. An exit interview was conducted with May Sabbah, Site Director. A copy of this report and Notice of Site Visit was provided. Appeal rights were issued and discussed.The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
