Licensing Program Analyst (LPA) Susann Sanchez conducted an announced complaint inspection to the above facility via Zoom due to the COVID19 Pandemic. LPA met with Juanita Clark, Licensee and delivered the findings of the complaint. LPA observed three school aged children member during the inspection. Licensee gave LPA a vitrual tour of the home, which began at 10:36am.

During the investigation LPA interviewed parents and children. LPA obtained a copy of the facility roster and copies of other supporting documentation. In the first allegation the complainant stated that hot food left on stove while unsupervised children present. Based on interviews with staff (2), parents (4), and children (4) there was no disclosures indicting licensee leaves stove on and unattended. Complainant then stated that stove was off but the stove was still warm. Based on interviews, the above allegation is unsubstantiated. Although the allegation may have happened or is valid, there is not preponderance of evidence to prove the alleged violation did or did not occur, therefore these allegation are unsubstantiated.

In the second allegation the complainant stated that the facility has hazardous items in children's play area. Complainant stated that the hazardous item is a folding bed. Per Licensee, folding bed has wheels and is removed once children start arriving to the facility. Based on interviews with staff (2), parents (4) and children (4) there was no disclosures indicting that there is a hazardous item in the living room. Based on interviews, the above allegation is unsubstantiated. Although the allegation may have happened or is valid, there is not preponderance of evidence to prove the alleged violation did or did not occur, therefore these allegation are unsubstantiated.

In the third allegation complainant stated that Licensee did not ensure children had safe drinking water. During interview with Licensee, Licensee stated that she gives the children bottled or facet water. There is no regulation against facet water. Based on interviews with staff (2), parents (4) and children (4) there was no disclosures indicting that the water is unsafe. Based on interviews, the above allegation is unsubstantiated. Although the allegation may have happened or is valid, there is not preponderance of evidence to prove the alleged violation did or did not occur, therefore these allegation are unsubstantiated.

Exit interview was conducted with Licensee Juanita Clark, via tele-inspection, during which appeal rights were explained. This report along with a copy of the appeal rights will be sent to the licensee via email with a read receipt or confirmation of receipt of email, which will act as the licensee's signature
