25 | Licensing Program Analyst (LPA), Dayna Chambers, conducted a Case Management Incident inspection to follow up on self-reported incident that occurred approximately 3:45pm on 01/29/20. The Monterey Park South West Child Care Office received the incident report on 01/29/2020 by Silvia Romero, Assistant Site Supervisor. Report stated that on 01/29/2020, two children were playing and running in opposite directions and bumped into each others foreheads. One child had a cut in his mouth that was bleeding. The child was taken to the doctor by his parents and then to the dentist for x-rays as a precaution. The child healed and there was no further injury. The second child had no marks or any injury. The supervision in the outdoors play area, all staff were placed on the playground in areas. There are 7 areas covered during playtime. The children were playing and ran into each other by accident. The center contacted the parents and notified them of the incident. The children are still attending the center.
LPA met with licensee and confirmed facility was properly cared for during the incident. On 02/19/2020 at 10:48am am LPA and licensee toured the facility, at the time of the inspection all ratios were compliant according to Title 22 Regulations.
Based on today’s inspection, and interviews conducted, the facility followed the appropriate Reporting Requirements, Notified Parents, and protocol was followed.
At this time based on the available information it does not appear this incident was the result of a Title 22 violation and the facility followed the appropriate regulations to care for the children in care.
The content of this report was read and discussed in detail at the time of with Director, Maria Cortez.
Upon receipt, Notice of Site Visit shall be posted. Notice of Site Visit shall be posted for thirty (30) days. Failure to maintain posting as required will result in a $100 civil penalty.
An exit interview was conducted, and a copy of this report was provided to Director, Maria Cortez. Notice of Site visit was issued. |
