Licensing Program Analyst (LPA) Dayna Chambers conducted an unannounced annual inspection on May 26, 2023, at 8:45am. LPA met with Maria Lopez, Director, who guided analyst on a tour of the facility. This is a preschool program which consists of 6 classrooms: RM #1 to RM#6. Facility operation hours are Monday to Friday from 7:00am to 6:00pm. There were 39 children present upon arrival. All areas identified on the Facility Sketch were inspected. Upon arrival, the following staff were present during this inspection: Room #1: Staff #1, #2 and with 11 preschoolers; Room #4: Staff #4, #5 and with 10 preschoolers; RM#5: Staff #6, #7 and with 7 preschoolers; RM#6: Staff #8, #9, with 11 preschoolers. Total of 39 children observed. Young Horizons Agency has 5 total centers. Central Pacific Center: 198006417 is currently utilizing two classrooms starting May 8, 2023, to approximately June 16, 2023. Central Pacific is currently renovating the playground. The center provided a written notice. Classroom RM#2: Staff #1, Staff #2 with 10 preschoolers; RM#3: Staff #3, #4 with 5 preschoolers. Total of 15 children observed. The facility was observed to be within the license capacity and limitations. The following was observed during the tour of the facility: Furniture and equipment were inspected for age appropriateness and in good repair. Telephone service, heating, lighting and ventilation were evaluated. Children have their own cubby to store their belongings. Linens are taken home each week to be washed. Napping equipment (mats) were observed in a separate storage room. Per director, the isolation area is in the office. Age-appropriate sinks and toilets were inspected for availability and good repair in all restrooms. General sanitation was observed. Availability of indoor drinking water was observed in classrooms. Disinfectants, cleaning solutions, medication and other items that are dangerous to children, were inaccessible to children. Director states that poisons are locked in the classrooms. Carbon monoxide detectors were observed and are operable. All kitchen areas/food preparation areas and food storage areas are kept clean and are free of litter, rubbish, rodents, and/or any other vermin. All storage containers for solid waste, including moveable bins shall have tight-fitting covers that are kept on, and in good repair. Trash cans used to discard food have tight fitting lids or solid waste bags shall be discarded immediately after each meal. Outdoor playground equipment is in a safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All areas around or under high climbing equipment, swings, slides, and similar equipment are cushioned with material that absorbs a fall. There is adequate shade in the play yard. drinking water was observed. LPA advised that no children shall be left without the supervision of a teacher at any time. All floors were observed to be clean and safe. All materials accessible to children were observed to be toxic-free There are no firearms stored on the premises. There are no pools or bodies of water at the facility. All individuals present have obtained a criminal record clearance or criminal record exemption. There is at least one person trained in CPR and Pediatric First Aid present during this inspection. Children’s Records were reviewed. (The name, address, and telephone number of child’s authorized representative is on file) and medical Assessment.) for completeness; Inspection of required forms was made and documented on the LIC 857. Staff present did have proof of the AB 1207 Mandated Reporter Training certificate on file. All staff have been given on the-job training sanitation principles, housekeeping, including universal health precautions. LPA did not review staff files during the inspection due to the staff files being stored at the administrative office. LPA will review staff files at a later time and date. If staff files were to be incomplete during the time and date that LPA reviews files, a case management inspection will occur at the facility to reflect the incomplete files. Children's roster was reviewed and is current. Sign-In and Sign-Out sheets were reviewed. Children presents were signed in. Disaster drill log was available, last drill was conducted on May 8, 2023. Menus are posted one in advance where it is visible by the child's authorized representative. Menus for the past 30 days are available upon request. Snacks were reviewed for availability, quantity and appropriateness to children in care. The facility provides Breakfast, Lunch and PM snack. The Kohn Administrative office provides the meals, and they are delivered to the centers. First Aid supplies were observed in the classroom in an emergency backpack. According to the Director, medication is only administered to a child when accompanied with a doctor's note and is stored in the office. The facility was observed to be equipped with an isolation area for any child who becomes ill, and it is in the office. The Director is updating the IMS Statement. Medication: This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. Director submitted the completed lead testing reports and licensing forms on 10/27/22. There were no lead exceedances and no corrections needed. volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

A notice of site visit was given and must remain posted for 30 days.
