1 2 3 4 5 6 7 8 9 10 11 12 13 14 15 16 17 18 19 20 21 22 23 24 25	On 9/27/2024, at 8:45 am, Licensing Program Analysts (LPAs) Carolyn Tuba and Mariah Aguirre conducted an unannounced Annual inspection. LPAs entered the facility which is a commercial building. The entrance door has a monitoring system with camera where staff have the ability to buzz families in. LPAs met with Director, Panfilia Gonzalez. LPAs were guided on a tour of the facility indoors and outdoors. Covid Risk was conducted. This is an infant program licensed for 12 children including a preschool component. The Center's days and hours of operation are Mon.-Thurs. 6:45 AM. to 6:45 PM. and Friday: 7.00 AM to 5.00 PM. The Director stated there are 12 children currently enrolled. The facility is located in a business commercial center and is on the lower floor of a high-rise building. LPAs observed a total census of 4 children with 1 staff member. The following infant classroom was inspected. All staff members are fingerprint cleared. Licensed facility is within the conditions, limitations, and capacity specified on the license. The facility has additional staff members to support and assist with the needs of staff and infants. The classroom furniture and equipment were inspected for good repair, free of sharp, loose, or pointed parts. Indoor infant classroom was inspected to ensure that the floors have a surface that is safe and clean. Telephone service, heating, lighting, and ventilation were evaluated. There is storage for children’s belongings. Infants have a designated sleeping area with enough cribs and the mattresses are firm and in good condition, as well as cots which are available for infants over a year old. Fitted sheets are supplied by the parents and are sent home to be laundered each day. LPAs observed the 15-minute sleep documentation, as well as the sleep, feeding and diaper changing logs. A copy is given to parents each day. Availability of drinking water was reviewed and per Director parents provide water from home and refills are available by the use of Sparklets bottled water. There is also filtered water stations. There is a changing table inside the restroom with arm’s length sink for hand washing and 1 toilet. Diapers and wipes are supplied by Page 1 of 4

parents, but extra supplies are available. Toys and surfaces are observed to be safe and clean. Staff restrooms are available in the hallways outside of the classrooms.

Combination carbon monoxide and smoke detectors were present in each classroom throughout the facility. LPAs observed while Director tested and found to be operable during the time of the visit. Fire extinguishers are serviced annually and were last serviced May 11, 2024. Storage cabinets in classrooms were inspected and no hazardous items were found, cleaning items are kept in the classroom cabinets, which are locked and inaccessible to children in care. No poisonous cleaning compounds were found in the childcare center. Emergency disaster drill was last conducted in August 2024. There is a First Aid kit available in the classroom and the school office; and supplies were inventoried and found to be in good condition.

There is a kitchen area where meals are prepared and distributed to each classroom. Food preparation areas were toured for safety, cleanliness, and proper equipment. The facility is part of a food program and provides meals for the 2 – 5-year-olds however the infant program meals are provided by parents including formula, baby food and all meals including Breakfast, Lunch and AM/PM snacks. Breast milk is brought in and stored in a fridge in the infant room with child’s name and date. LPAs observed parent board with all required documentation located in the entry way of the facility. Infant classroom also has additional documentation on the parent board.

LPAs were informed that there are currently no children on medication at this time, however the Director has an IMS plan and LPAs reviewed and provided consultation. Director stated that medication would be kept in a medication lockbox and staff is aware where it is kept and how to administer. LPAs advised that medication should be given back to parents/guardians when child leaves the facility or medication expires.

Outdoor playground equipment is in safe condition, free of sharp, loose, or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. There are currently 2 playgrounds, and 1 playground is designated for the infant component. There is a waiver for staggered play times for the 2 years olds to use the infant playground. The playground has age appropriate equipment with a cushion surface or falls. There is adequate shade in the play yard with awnings over the play equipment. There is enough bikes and toys for the children to use in the play yard.

Sign in/out sheets are located at the front entrance of the facility. The LPAs reviewed and reminded Director to have parents use full signatures.

LPAs reviewed staff records and found that they were complete. Clearance for adults and verification of CPR/First Aid, educational requirements, immunization records and health preventative practices documentation was reviewed. Children’s records were reviewed to ensure that Identification and Emergency form and a medical assessment are on file. LPAs observed Infant Sleep Plan for children who were enrolled prior to their first birthday. There is also a Needs and Service Plan conducted when entering the program, every 3 months and as needed. LPAs advised Director that at least herself or one staff member is required to have the 8-hour Health, Safety, Nutrition and Lead training. Technical Violation was issued.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Childcare Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For childcare center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP). LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP. LPAs reminded that lead results need to be posted for parents to view until the next testing.

Director was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

AB1207 Mandated Child Abuse Reporting – On or before March 30, 2018, any person who works in a childcare facility shall complete the training and renew the training every 2 years. LPAs discussed the safe sleep regulations with the Director and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee [or facility representative] of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Director was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California.

At this time, the facility is in compliance with California Code of Regulations Title 22. No deficiencies are being cited.

A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the Director, Panfilia Gonzalez.
