Licensing Program Analyst (LPA) Raul Navarro conducted an unannounced Annual Random inspection. LPA met with facility Licensee, Linda Johnson, who guided LPA on a tour of the facility. This is a preschool program licensed for 75 children which operates Monday – Friday from 6:30 AM – 6:00 PM. Per the Licensee there are 55 children enrolled.

All areas identified on the facility sketch were inspected. The program consists of five classrooms. Upon arrival, the following staff were present during this inspection: Room 1: Staff #1, with 7 children; Room 2: Staff #2 with 8 children; Room 3: Staff #3 with 10 children; and Room 4: Staff #4 with 7 children. The following was observed during the tour of the facility:

Teacher-child ratios were observed to be in accordance with Title 22 regulations. Staff names were recorded. All children were observed to be under visual supervision of a teacher at all times. LPA observed all required forms/publications to be posted and visible to children's authorized representatives. The Licensee is within the conditions, limitations, and capacity specified on the license.

Furniture and equipment was inspected for good repair, free of sharp, loose, or pointed parts. All indoor classrooms were inspected to ensure that the floors have a surface that is safe and clean. Per Licensee, there are currently no children on medication. All toilets and hand washing facilities are in safe and sanitary operating conditions. All materials and surfaces accessible to children are toxic free. At this time, the office is used as an isolation area. There are chairs in the office and a mats are also readily available . Parents are contacted immediately when children are determined to be ill. First Aid supplies are available and complete. Sufficient individual storage space for children was observed. Each child has a cubby. LPA observed sufficient napping equipment.

Snack menus were reviewed to ensure that they are being posted at least one week in advance and visible to an authorized representative. The facility provides AM snack, and PM snack. All kitchen, food preparation, and storage areas are clean, free of litter, rubbish, and rodents/vermin. Children bring their own lunch from home in labeled lunch pails/bags. All food is protected from contamination. There is drinking water available in all indoor classrooms and outdoors. All storage containers for solid waste, including moveable bins, have tight fitting covers on and are in good repair. Disinfectants, cleaning solutions, poisons and other items that are dangerous to children are stored in an area inaccessible to children.

Facility has one or more functioning carbon monoxide detectors that meet statutory requirements. The licensee takes measures to keep the facility free of flies, other insects and rodents. Outdoor play equipment was observed to be in good condition, free of sharp, loose or pointed parts. Outdoor activity space surface is maintained in a safe condition as is free of hazards. The Director states that there are no bodies of water on the premises and LPA did not observe any bodies of water during this visit. Areas around and/or under climbing equipment, slides have a cushioning material to absorb a fall. Licensee states there are no weapons or firearms on the premises.

Sign in and out sheets were reviewed to ensure that the person who signs the child in and out uses their full legal signature and records the time of the day. A random sample of Children’s Records were reviewed to ensure that they are complete. LPA issued the Children’s Record Review (LIC 857) to the Licensee during this inspection. In review of children’s records, files contain information including, but not limited to the following: Name, address and telephone number of the child's authorized representative and of relatives or others who can assume responsibility for the child if the authorized representative cannot be reached when necessary.

Staff records were reviewed. All individuals present have obtained a criminal record clearance or criminal record exemption. There is at least one person trained in CPR and Pediatric First Aid present during this inspection. SB792 Immunization Requirements for Staff and Employees was discussed with the Licensee. The Licensee and her staff have current immunization documentation on file. AB1207 Mandated Child Abuse Reporting – Implementation was discussed with Licensee. Website http://mandatedreporterca.com. Staff present have not completed the Mandated Reporter Educators Training; certificates are on file.

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. LPA advised the Licensee to access forms, regulations and quarterly updates on the Child Care Licensing website at: www.ccld.ca.gov.



The deficiency listed on the following page were observed by the LPA and are being cited in accordance with California Code of Regulations Title 22. Please see attached LIC 809-D. Deficiency that is being cited needs to be cleared to protect the children’s health & safety.

Exit interview was conducted with Licensee. The Licensee was provided a copy of their appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these forms.

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site inspection by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
