13 | Licensing Program Analyst (LPA) Judy Mora conducted a subsequent complaint inspection to conclude the investigation in regards to the above complaint allegation. LPA met with Teacher, Michelle Uricar, as Site Supervisor, Maral Martirosyan was not present. LPA observed 27 children with 05 staff at the time of inspection.
During the course of this investigation, interviews were conducted with the Site Supervisor, facility staff and children. LPA also reviewed and obtained copies of the child's ID and Emergency Information.
Although the allegation may have happened or is valid there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is unsubstantiated. There were no disclosures made by staff or children in regards to the allleged allegation.
The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. Exit interview conducted with Michelle Uricar. Appeal Rights were provided. |
