Licensing Program Analyst (LPA)Katrina Chicote and Licensing Program Manager (LPM) Trevino Cochran conducted an Unannounced Required Annual inspection to the above facility on 10/01/2021 at 9:15 AM. LPA conducted COVID-19 Screening Call prior to entering facility. Upon arrival, LPA disclosed the purpose of the inspection and met with Licensee, Claudia Romero, who guided the LPA on a tour of the facility. Also present were two other adults, who live in the home and have criminal record clearance. LPA observed two other adults working with children as Licensee's Assistants. There were seven day care children present in the beginning of today’s inspection and eight total. Licensee states that there are currently eleven children enrolled. The children's roster was reviewed and is current. Per Licensee, the facility’s hours of operation are Mon thru Fri, 7am to 5pm. Emergency Disaster Plan, License, and Parents’ Rights were posted at the time of inspection. Disaster drill log was also available during today’s inspection, last disaster drill conducted on 09/31/2021.

This is a single-story home which consists of two bedrooms and two bathrooms, kitchen, living room, and detached converted garage that is being used for day care activity space. LPA observed an apartment above the garage, Licensee states that apartment has different address from facility (735 1/2 E. 92nd St.) and individuals residing in apartment has criminal record clearance. LPA confirmed individuals have criminal record clearance. Per Licensee, areas off limits to children and parents include Bedroom 1, Bedroom 2, and Bathroom 2. LPA observed signs posted on doors stating area is off limits and doors to be closed at time of inspection. Licensee stated Living Room in the home will be used for napping, and kitchen and dining room is only used as a walkway for children to have access to Living Room for nap time. LPA observed operable child safety locks on kitchen cabinets and drawers at time of inspection. At 9:34 AM, LPA observed blue cots being stored in Kitchen and LPA observed large sectional couch in Living Room and ceiling fans all through out inside of the home. Per licensee, off limit areas are also locked during operating hours.

All areas identified on the facility sketch as accessible to children were inspected to ensure that they are clean

and orderly with ventilation and heating for the safety of the children. At 9:30 AM, LPA observed converted
detached garage that Licensee states will be used for day care activities only and not used for napping and eating. LPA observed carpet through out, wooden play kitchen, wooden shelves filled with plastic manipulatives, puzzles, wooden blocks, and other age appropriate toys free of lose and sharp parts available for children in care at time of inspection. At 9:32 AM, LPA observed Bathroom 1, that will be used by children, through a door in the garage to the right, to have adequate hand washing supplies, operable sink and toilet, a diaper changing table, and cubbies filled with diapers and wipes inside Bathroom 1. There is a working telephone maintained in the home. Per Licensee, she has two dogs that is kept behind double gate in rear of property. LPA observed double gate and dog house. Detergents, cleaning compounds, medications, and other items which can pose a danger to children are inaccessible. At 9:36 AM, LPA observed these items stored in a high cabinet on top of laundry area and sharp knives kept in a high cabinet in kitchen making these items inaccessible to children in care at time of inspection.

Per Licensee, the children will have access to backyard area for outdoor play and eating. LPA observed backyard has adequate perimeter fencing through-out the yard, turf grass, and soft black cushioning material on the floor. LPA observed backyard to be accessible through sliding glass door in daycare activity space. LPA observed cubbies for children's belongings, child size tables and chairs, and a two story plastic play structure with slide that has soft cushioning below to absorb falls, and other age appropriate toys free of loose and sharp parts available for the children in the backyard and all trees, shrubs, and plants are maintained at time of inspection.

The valve on the required 2A 10BC fire extinguisher indicates fully charged and was being serviced at time of inspection. At 9:40 AM, LPA observed smoke and carbon monoxide detectors are in operable condition at time of inspection. Per Licensee, First Aid kit is kept in the daycare activity space and outdoor play area, LPA observed First Aid kit hanging in outdoor area and inside daycare activity space. The Licensee has current Pediatric First Aid and CPR. Proof of immunization against influenza, pertussis, and measles was readily available during today’s inspection. The Licensee has also taken the Mandated Reporter Training

Per Licensee, there are no weapons, firearms in the home and there are no bodies of water around the premises. LPA observation did not see any bodies of water around the premises or firearms at time of inspection. Per Licensee, they provide food for children in care. Licensee states sick policy is updated and sick isolation area for children is inside until child is picked up by authorized representative.

The following were discussed:
Incidental Medical Services (IMS)
Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Family Child Care Homes Section 102417. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Licensee (or facility representative) was reminded that all adults 18 and over living or working in the home, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Rooms that are off-limits need to be made inaccessible during operating hours. NO smoking, NO infant walkers, NO Johnny jumpers, NO saucer chairs, NO incline sleepers and any other item that falls into that category are permitted in the facility. Effective January 1, 2010, licensees of family child care homes are required to ensure that at least one staff member with current training in pediatric first aid and pediatric CPR is on site at all times when children are present.



The Licensee was advised that inaccessibility of hazards must be constantly reassessed depending on the children in care. Sudden Infant Death Syndrome (SIDS) and Never-Shake-a-Baby were discussed. A hard copy of PIN 21-25-CCP, PIN 20-24-CPP was provided.

AB 1207: Beginning on January 1, 2018, this law requires all licensed providers, applicants, directors and employees to complete training as specified on their mandated reporter duties and to renew their training every two years. Volunteers are encouraged but not required to take the training. Website: http://www.mandatedreporterca.com/

Senate Bill 792: this bill, commencing September 1, 2016, prohibits a person from being employed or volunteering at a child care facility or family day care if he or she has not been immunized against influenza, pertussis and measles. LPA discussed the influenza waiver during the visit.


The facility was found in compliance per Title 22 regulations, there will be no deficiencies cited today 10/01/2021.

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site inspection by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
