Licensing Program Analyst (LPA) Elka Chavez conducted an unannounced 1-year required inspection. Facility is currently licensed for a capacity of 80 children. LPA met with Lead Teacher, Jose Reyes as Director Leticia Rico Nunez was unavailable at the time of inspection. The program currently operates Monday thru Friday from 8:00am to 2:30pm. Licensing staff was taken on a guided tour of the facility of both indoors and outdoors.

At 9:30 AM all areas identified on the facility sketch were inspected. Upon arrival, LPA observed 0 children. Furniture and equipment were inspected for good repair, free of sharp, loose, or pointed parts. All indoor classrooms were inspected to ensure that the floors have a surface that is safe and clean. All toilets and hand washing facilities are in safe and sanitary operating conditions. All materials and surfaces accessible to children are toxic free. At this time, the off limit single restroom is used as the designated ill isolation are. Parents are contacted immediately when children are determined to be ill. A first aid kit is kept in each classroom. Smoke detectors are present and fire extinguishers have been serviced in July 2021.

Snack menus were reviewed to ensure that they are being posted at least one week in advance and visible to an authorized representative. Food is provided by an outside vendor. The facility provides breakfast, lunch and PM snack. All kitchen, food preparation, and storage areas are clean, free of litter, rubbish, and rodents/vermin. Children have their own personal drinking containers that are labeled with their name. All storage containers for solid waste, including moveable bins, have tight fitting covers on and are in good repair. Disinfectants, cleaning solutions, poisons and other items that are dangerous to children are stored in an area inaccessible to children. LPA observed child safety latches on the storage cabinets in each classroom. Storage areas for poisons are locked. Facility has one or more functioning carbon monoxide detectors that meet requirements.

Outdoor activity space equipment and surface is maintained in a safe condition as is free of hazards. Areas around and/or under climbing equipment and slides have cushioning material to absorb a fall. LPA did not observe any bodies of water during this visit.

At 11:15 AM, LPA checked licensing automated system for any incident reports regarding the closure of the facility starting 6/13/22 through 6/22/22 and did not see one on file. The facility representative, Jose Reyes was unable to obtain a copy of the incident report for LPA.

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Facility representative, Jose Reyes was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Based on the LPA’s observations and records review, the following deficiencies listed on the attached LIC 809 (deficiency page) are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children’s health & safety.
