At 9:25 am Licensing Program Analyst (LPA) Roxana Lopez conducted an unannounced required 1 year inspection to the above facility. A risk assessment was conducted upon entry- appropriate PPE was used. LPA met with America Alcantar , Licensee who guided analyst on a tour of the facility. Per Licensee, there are currently 9 children enrolled. A current children’s roster was available for review. There were 8 children present upon arrival- 3 being infants. Also present during inspection was Assistant Arielle Cazarin.

Upon entry of the home at 9:25 am LPA observed child # 1 infant sleeping in a car seat. Per Licensee, child arrived at 9:00 am. LPA advised that car seats should only be used for transportation and asked for child to be removed from car seat. This is an immediate risk(s) to the health, safety, or personal rights of child in care. LPA observed Child # 3 waking up in a playpen. Child # 3 was removed from playpen by assistant and Child # 1 was placed in playpen to continue sleeping. LPA advised that each infant enrolled should each have appropriate sleeping arrangements and their own bedding while in care.

The licensee states that 2 adults currently live in the home. Per Licensee, they currently have 1 assistant. All adults present in the home have obtained a criminal record clearance or exemption prior to working, residing or volunteering in the licensed child care home. Licensee states that there are no firearms/ weapons stored in the home.
This is a single- story home which consists of 3 bedrooms, 2 bathrooms, living room, kitchen, laundry room, garage and backyard (fenced). The children have access to living room, kitchen, and 1 bathroom, garage (for activities) and backyard fenced. ------------------------ pg. 1 of 5 --------------------

Per licensee, areas off limits to children and parents include, 3 bedrooms, 1 bathroom and laundry room. The LPA toured all areas used by children during this visit.

All areas identified on the facility sketch that are accessible for children to use were inspected for safety, comfort, and cleanliness. There is telephone service via a cellphone that stays at the facility during operation hours. There is ventilation and heating (central). The following was observed and reviewed during this inspection.

Detergents, cleaning compounds, and medications were observed to be inaccessible to children. Per Licensee, there is no poisons in the home and understands that poisons need to be locked under key. The restroom that children use was observed to be safe and sanitary.

LPA Lopez did not observed a fireplace. The valve on the required 2A 10BC fire extinguisher indicates fully charged and was serviced on 08/1/2022, as indicated on service tag. Smoke and carbon monoxide detectors were tested and are operable. All homes shall conduct fire and disaster drills at least once every six months, and document the date and time of each drill. Last drill documented was conducted on 3/8/2023.

Emergency Disaster Plan, Parent’s Rights Poster and the Facility License were observed to be posted in the garage( activity area) .

The home is observed to be clean and orderly. There are toys available for children. Sleeping arrangements in form of mats and 1 playpen.

At 9:40 am LPA did observe an infant walker in the garage (activity space). Per Licensee, the walker belongs to her grandchild who is not part of the daycare. LPA advised that when grandchild is present in the home they count as part of their daycare and all regulations apply.

Licensee states that they are currently caring for infants. LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

At 10:15 am LPA observed that infants enrolled do not have the LIC 9227 Individual Sleeping Plan on file and no sleep log documenting infant’s sleep every 15 minutes.

Smoking is prohibited in a licensed Family Child Care Home. Per Licensee, no one smokes in the home.

Currently, children are using the back yard for outdoor play time. The outdoor play area was observed to be fenced. LPA observed that the outdoor yard has toys and other materials for children to play with. LPA did not observe any objects that can pose a danger to children on the outdoor yard.

There is two dogs in the facility. Per Licensee, dogs is inside the house during day care hours.

The licensee is observed to be operating within the license capacity limitations.

At 10:15 am children’s records were reviewed, including emergency information. LPA observed that 3 out of 7 files reviewed did not have proof of immunizations.

The licensee and other personnel have completed training on preventive health practices including Pediatric First Aid and CPR. The licensee's and assistant’s Pediatric First Aid and CPR expires on 6/2023. There are first aid supplies available.

LPA observed that the Licensee and assistant do have proof of immunization record and the Mandated Reporter AB 1207 compliant Child Care Training Certificate on file.

Licensing staff consulted with licensee. regarding COVID-19 health and safety guidelines on this date,

Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Based on the LPA’s observations and records review, the following deficiencies listed on the attached LIC 809D (deficiency page) are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children’s health & safety.
