25 | Licensing Program Analysts (LPAs) Crystal Green and Bardo Baluyot conducted an unannounced case management licensee initiate inspection today. Upon arrival licensing staff met with Yesenia Urbina, Licensees’ Assistant, until Licensee Gladys Ortega arrived shortly after. Due to licensee primary language being Spanish the licensee assistant assisted with translation. The licensee is requesting a capacity increase from a small family child care home license to a large family child care home license. The licensee states that she currently has 9 children enrolled. A current children’s roster is available and is current. Licensee also has a Foster License (197806197).
This is a single story, four bedroom, two bathroom home. Currently residing in the home are three adults (licensee, adult daughter Yesenia Urbina, and adult son Osvaldo Sanchez) and 1 minor child. Areas accessible to children were inspected as follows: Living room, dining area, one bathroom, one bedroom, den area, and back yard (fenced). Areas off limits include: Kitchen, three bedrooms, one bathroom, front yard and attached garage.
There are 2 pet parrots (caged) located outside in the off-limit area and 3 dogs which are kept in the off-limit area indoors. Licensee states that she currently has only one assistant. All individuals present in the home have obtained a criminal record clearance or exemption prior to working, residing or volunteering in a licensed home. Detergents, cleaning compounds, medications, and other items which can pose a danger to children are inaccessible in all areas of the home. The licensee states that there are no poisons in the home. The licensee does understand that poison must be locked with a key or combination lock.
Per licensee, there are no weapons, firearms or bodies of water on the premises. There were safe toys, play equipment and materials observed for children. Emergency Disaster Plan, Parent’s Rights Poster and the Facility License are observed to be posted. Children’s records were reviewed to ensure that each child has an Identification and Emergency form and Consent for Medical Treatment on file. |
