Licensing Program Analyst (LPA) Elka Chavez conducted a case management inspection to the above named facility. LPA met with Assistant Director, Denisse Cardenas who assisted LPA with touring the facility.

At 12:00 PM LPA toured the facility. LPA did not observe Covid-19 posters throughout the facility. LPA observed 5 of the 7 staff present were not wearing facial coverings inside the facility as recommended by the Los Angeles County Health Department. LPA observed children age 2 and older were not wearing facial coverings inside of the classrooms. At 3:30 PM LPA Chavez provided Covid-19 Technical Assistant to Assistant Director, Denisse Cardenas.

Exit interview was conducted with Denise Cardenas.



A notice of site visit was given and must remain posted for 30 days.
