An unannounced Annual Required Inspection was conducted on 06/03/2021 at 9:15 AM by Licensing Program Analysts (LPAs) Katrina Chicote and Randy Derraco. Facility is currently licensed for a capacity of 25.. Licensing staff met with Assistant Director and Director arrived after the tour at 9:45 AM. The program currently operates Monday thru Friday from 8am to 4pm. LPAs were taken on a guided tour of the facility of both indoors and outdoors.

At the initial start of the site visit, there is a total census of 16 children and a total census of three teachers present. The facility is made up of four classrooms, the 2s Classrooms, 3s Classroom, Preschool, and School-Age. The facility is also attached to an Infant Center (198014887). At 9:15 AM, LPAs toured 2s Classroom, furniture and equipment were inspected for age appropriateness and good repair, first aid kit observed in classroom hanging by door way. LPAs observed bathroom to be free of hazards and toilets and sinks in operable condition at time of inspection. At 9:16 AM, LPAs toured 3s Classroom, furniture and equipment were inspected for age appropriateness and good repair, first aid kit observed hanging by doorway. LPAs observed bathroom to be free of hazards and toilets and sinks in operable condition at time of inspection. At 9:18 AM, LPAs toured school-age classroom where four children and one Staff 1(S1) were sitting on desks doing online learning. At 9:20 AM, LPAs observed Preschool Classroom with twelve children and one staff member (S3). LPAs observed furniture and equipment were inspected for age appropriateness and good repair, first aid kit observed in classroom by doorway. LPAs observed bathroom to be free of hazards and toilets and sinks in operable condition at time of inspection. Telephone service, heating, lighting and ventilation were evaluated. There were cubbies observed as storage for children's belongings. Mats/cots were inspected. Per Assistant Director, linens are provided by the facility and washed weekly by facility. Availability of drinking water was reviewed, LPAs observed bottled waters in refrigerator in Preschool Classroom. Per Assistant Director, children also use their own personal water bottles. Age appropriate sinks and toilets were inspected for availability, good repair, water temperature, toilet paper,

area safety and sanitation. Toys kept in cubbies in each classroom were observed to be clean at time of inspection.

Carbon monoxide detectors and smoke detectors are present in the facility. At 9:25 AM, LPAs tested carbon monoxide and smoke detectors to be operable at time of visit. Fire extinguishers have been serviced in 11/12/2020. Hazardous items including poisonous cleaning compounds were stored inaccessible to children.

Snack/lunch menus were reviewed. Food and snacks were reviewed for availability, quantity and appropriateness to children in care. At 9:36 AM, LPAs toured food preparation areas for safety, cleanliness and proper equipment.

At 9:40 AM, Outdoor equipment was inspected for safety, cushioning material, good repair and age-appropriateness. LPAs observed large metal play structure with two plastic slides and monkey bars with blue cushioning material at the bottom. LPAs also observed wooden play house, bicycles, and other age appropriate toys to be in good repair at time of inspection. Required shade and fencing were inspected. LPAs Chicote advised Assistant Director that the children need to be within the direct care and supervision, including visual supervision of the teacher(s) at all times. Per Assistant Director, they bring children's water bottles out during outside play. Play area was inspected for hazards and inaccessibility to bodies of water; no bodies of water or hazards observed at time of inspection.

Teacher-child ratios were observed and staff names recorded. Care and supervision was evaluated to determine if the basic needs of children are met and appropriate. Sign-in and out sheets and procedures were reviewed. Personal Rights of children were observed by LPA.

Staff and Children’s Records were reviewed. Criminal Record Clearances were reviewed for adults. Pediatric CPR card expires in 08/20/2021 for Assistant Director. Inspection of required forms made.

Incidental Medical Services:
Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. The following were discussed with the Director:
Rooms that are off-limits need to be made inaccessible during operating hours. Smoking is prohibited. No infant walkers, no Johnny Jumpers, no excersaucers or any other item that falls into that category are allowed in facility.

The Director was advised that inaccessibility of hazards must be constantly reassessed depending on the children in care. Sudden Infant Death Syndrome (SIDS) and Never-Shake-a-Baby were discussed. LPA also provided a complete packet of the Provider Information Network (PIN) 10-02-CCP dated February 20, 2019 regarding Safe Sleep Awareness Campaign during today’s inspection.

The fire extinguisher type 2A-10BC must be serviced annually or as often as necessary. Smoke and Carbon Monoxide detectors should be checked and batteries replaced as needed. Changes should be reported to the Department as soon as they occur such as construction, remodeling, telephone number changes and/or if you move from your location.

Mandatory Forms for the children’s files and staff files, requirements for fire drills, earthquake drills and documentation were discussed. Role and responsibilities of being a Mandated Reporter were reviewed. The Director was advised how to access forms and Regulations online at www.ccld.ca.gov. Director was made aware that it is his/her responsibility to know the regulations as well as anyone who assists in providing care. The Director was advised that inaccessibility of hazards must be constantly reassessed depending on the children in care.

LPA advised Director that all adults 18 years of age and older providing Care & Supervision and/or have continuous presence in the facility shall adhere to a criminal background clearance with the Department of Justice, FBI and Child Abuse Index Check.

LPA informed Director to log onto web site www.ccld.ca.gov to obtain forms and LIVESCAN application. Records for all children and staff must be maintained for three (3) years after separation from the facility.

The Director was also advised of the requirement to report Unusual Incidents and/or injuries to the parent/guardian and to CCL within the time frame specified by the regulation. Director advised to visit www.shotsforschool.org for immunization information.

The facility was found in compliance per Title 22 regulations, there will be no deficiencies cited today 06/03/2021.

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site inspection by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.



Exit interview was conducted with Director, Leticha Toney, on 06/03/2021 at 11:00 AM. A copy of this report was given and appeal rights were issued and discussed.
