On Monday, June 5th, 2023, at 9:35 a.m. Licensing Program Analyst (LPA) Mayra Rivera conducted an unannounced annual inspection and met with director Latanya Hardy who guided LPA Rivera on a tour of the facility.

This is an infant program that consist of one classroom and one outdoor play yard. This program serves children ages 12 months to 2 years. This program operates Monday – Thursday 6:30 a.m. - 3:30 p.m and Friday 6:30 a.m. - 2:30 p.m. Sign in and out takes place in the main entrance area. All areas identified on the facility sketch were toured and inspected both indoor and outdoor.



At approximately 9:35 a.m., LPA observed on the bulletin wall the license, LIC 9149 Earthquake Checklist, Parents Right's, Personal Rights, PUB 269 Child Passenger System Poster, LIC 610 Emergency Disaster Plan, monthly Menu and Daily Schedule. Side wall, LPA observed the facility map. LPA also reviewed the children's roster, children and staff files and the sign in/out sheets. LPA observed 1 signature and 1 child present today.

At approximately 9:43 a.m. LPA entered the infant classroom. LPA Rivera observed 1 infant with staff 1. LPA Rivera observed age appropriate furniture and equipment to be in good condition, free of sharp, not loose, or pointed parts and age appropriate toys accessible to children. LPA observed cubbies with children’s personal items, changing table arm’s length to a handwashing sink. LPA observed extra clothes and diapers in the cubbies near the changing table area. LPA observed cleaning solutions stored inside the closed cabinet with door closed and locked making it inaccessible for children to open. LPA Rivera observed trash bins with a tight-fitting lid for trash and diapers.

Heating lighting and ventilation were evaluated by LPA Rivera. LPA observed AC/heater and temperature at 71 degrees. LPA observed the fire extinguishers located in the hallway, and the valve on the required 2A10BC fire extinguishers indicating fully charged and serviced on 4/22/22. LPA informed the fire extinguishers are to be serviced yearly or purchase a new one. LPA Rivera observed fire alarm system. At approximately 9:56 a.m., LPA observed the carbon monoxide in the classroom. LPA tested the carbon monoxide alarm and is operable. LPA observed the first aid kit located in the classroom fully stocked with band aids, gauzes, adhesive bandages and antiseptics. LPA observed the fire/earthquake log and last drill conducted on April 25, 2023, and May 23, 2023. Lead testing was conducted on 2/10/22. For ill isolation children use nurses office.

At approximately 10:09 a.m. LPA Rivera entered the outdoor playground. LPA observed the infant area and equipment to be in safe condition, free of sharp, no loose or pointed parts. LPA observed two sheds closed and locked with a child proof lock. LPA did not observe bodies of water. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards and fenced all around. All areas around or under climbing equipment, are cushioned with granulated rubber that absorbs a fall. For outdoor water drinking a water pitcher is brought out and children are provided their own sippy cups and disposable cups. For adequate shade, LPA observed a tree and picnic table with an umbrella. LPA Rivera asked if there are any poisons, firearms, weapons, or bodies of water. director stated she has no firearms, or weapons nor water bodies of water. LPA did not observe firearms or weapons, poisons, nor bodies of water.

LPA did not observe bottles or formula because children enrolled in this facility are ages 12 months to 2 years and eat table food. Meals and snacks are provided by the facility. For water drinking LPA observed a water pitcher and per director disposable cups or sippy cups are provided to children. LPA observed the food preparation area to be clean and free of litter, rubbish, rodents, and/or any other vermin and storage containers for solid waste, including moveable bins with tight-fitting covers that are kept on, and in good repair. The facility was observed to be free of flies, other insects and rodents. LPA observed the refrigerator temperature to be at 36 Fahrenheit.

Staff present during this visit have obtained a criminal record clearance or criminal record exemption. There is at least one person trained in Pediatric First Aid/ CPR present during this inspection. At approximately 10:34 a.m., LPA Rivera reviewed children files to be incomplete and stated on LIC 857 Children’s Records. Staff files are incomplete and stated on LIC 859 Review Staff records. During today’s inspection, per director there is no children with food allergies or on medication.

Director Latanya Hardy has proof of pediatric First Aid/CPR certification (expires 8/20/23), preventative health and safety certification dated 6/22/20, and immunization against MMR, pertussis, and influenza declination. Missing AB 1207 Child Abuse Mandated Certification and the infant/toddler units. Director has been informed mandated reporter training must be completed every 2 years, and is available at www.mandatedreporterca.com

Items discussed during this visit:

1. The following items are an immediate violation: Refused Entry to a Facility or Any Part of a Facility is a violation of Section 1596.852, 1596.853 or 1597.09. Regulations 101238 (g) (2), The Presence of an Excluded Individual, Fire Clearance Violations, Accessible Bodies of Water, Accessible Firearms, Ammunition or Both

2. Breakfast/Lunch/Snack Menus: Menus are required to be posted one week in advance where it is visible by the child's authorized representative. Menus for the past 30 days must also be available upon request.


3. Current Children’s Roster: Each child day care facility shall maintain a current roster of children who are provided care in the facility. The roster shall include the name, address, and daytime telephone number of the child's parent or guardian, and the name and telephone number of the child's physician. This roster shall be available to the licensing agency upon request.
4.LIC 311A - Records to be Maintained at the Facility: LPAs advised that forms, regulations and quarterly updates can be accessed on the Child Care Licensing website at: www.ccld.ca.gov.
5.Pediatric First Aid and CPR: At least one person trained in Pediatric First Aid and CPR must be present.
6.Designated Staff: The name of the childcare center director or fully qualified teacher(s) designated to act in the director's absence must be on file.
7.Qualifications: Educational background, training, and/or experience for each staff present must be available for review.
8. Immunization Requirements: Commencing September 1, 2016, a person shall not be employed or volunteer at a day care center if he or she has not been immunized against influenza, pertussis, and measles
9.Children’s Records: must be available for review; including but not limited to, the following: Name, address and telephone number of the child's authorized representative and of relatives or others who can assume responsibility for the child if the authorized representative cannot be reached when necessary.

10.Incidental Medical Services (IMS): This facility provides Incidental Medical Services – IMS. For IMS information, see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. A Plan of Operation that includes IMS must be submitted to the Department. (An IMS Plan was provided with the application and will be reviewed).

Director Latanya Hardy was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPA discussed the safe sleep regulations with director Latanya Hardy and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed director Latanya Hardy of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

During this visit 2 Type B Violations were given for staff # 3 missing AB 1207 Child Abuse Mandated Reporting Certification and Infant/toddler units. One technical violation for not having parent consent for diaper ointment.

A notice of site visit was given and must remain posted for 30 days.
