An unannounced Required – 1 Year Inspection was conducted on 10/22/2021 at 8:57 AM by Licensing Program Analyst (LPA) Katrina Chicote. Facility is currently licensed for a capacity of 47. Licensing staff met with Alma Guzman, Director. The program currently operates Monday thru Friday from 6:30 AM to 6:00 PM. LPA was taken on a guided tour of the facility of both indoors and outdoors.

At the initial start of the site visit, there is a total census of 21 children and a total census of six teachers present. The facility is made up of two classrooms, Room 1 for Preschool age and Room 2 Toddler Room. Furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting and ventilation were evaluated. At 9:15 AM, LPA observed Room 1 to have cubbies for children's belongings, reading area, wooden shelves with puzzles, blocks, dolls, dramatic play, and other age appropriate toys free of lose and sharp parts available for children in care at time of inspection. LPA observed Room 1 floors to have laminate flooring and carpet in learning stations and to be free of hazards at time of inspection. At 9:17 AM, LPA observed Bathroom 1, which is a jack and jill bathroom shared with Room 2, to have three operable toilets, a fourth toilet currently out of order and covered, and four operable sinks, floor free of hazards, adequate handwashing supplies, and handwashing posters posted at time of inspection. At 9:18 AM, LPA observed diaper changing table with steps by entrance to Room 1. Director described diaper changing procedure to LPA. At 9:22 AM, LPA observed Room 2 to have to have cubbies for children's belongings, reading area, wooden shelves with puzzles, blocks, dramatic play, and other age appropriate toys free of lose and sharp parts available for children in care at time of inspection. LPA observed Room 2 floors to have laminate flooring and carpet in learning stations and to be free of hazards at time of inspection. Mats/cots were inspected. Per Director, linens are provided by the family and taken home on Fridays for washing and returned on Monday. Availability of drinking water was reviewed, LPA observed each classroom to have water filtration system, per Director teachers provide paper cups to children and obtain water from filtration system. LPA observed paper cups available in each classroom.

A first aid kit is kept in a high cabinet in each classroom, LPA observed first aid kit in each cabinet and inventoried for necessary supplies. Carbon monoxide detectors and smoke detectors are present in the facility. Fire extinguishers have been serviced in 08/17/2021. Hazardous items including poisonous cleaning compounds were stored inaccessible to children.

Snack/lunch menus were reviewed. Food and snacks were reviewed for availability, quantity and appropriateness to children in care. Food preparation areas were toured for safety, cleanliness and proper equipment. Facility provides AM snack, Lunch, and PM snack to children.

At 9:25 AM, LPA observed outdoor play area which is located upon entry to facility. Outdoor equipment was inspected for safety, cushioning material, good repair and age-appropriateness. LPA observed child size tables and chairs through down outside hallway which was being used for outdoor circle time for preschool age classroom at time of inspection. LPA observed building toys, free of lose and sharp parts, and plastic play structure with soft cushioning material at the bottom. LPA observed a plastic fence separating outdoor play area between Room 1 and Room 2. Per Director teachers fill up water pitchers using the water filtration system in each classroom and provide paper cups to children, LPA observed large water pitchers and paper cups available for children outside at time of inspection. Required shade and fencing were inspected. LPA Chicote advised Director that the children need to be within the direct care and supervision, including visual supervision of the teacher(s) at all times. Play area was inspected for hazards and inaccessibility to bodies of water; no bodies of water or hazards observed.

Teacher-child ratios were observed and staff names recorded. Care and supervision was evaluated to determine if the basic needs of children are met and appropriate. Sign-in and out sheets and procedures were reviewed. Personal Rights of children were observed by LPA.

At 10:35 AM, LPA reviewed twelve children's records and six staff files. Staff files are kept in separate location. At 11:30 AM, staff files were brought to facility and LPA was able to review six staff files. Criminal Record Clearances were reviewed for adults. CPR card expires in 08/2023 for Staff 6, which is the opener and 12/2021 for Staff 3 which is the closer. Inspection of required forms made.

Incidental Medical Services:
This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. The following were discussed with the Director:
Rooms that are off-limits need to be made inaccessible during operating hours. Smoking is prohibited.

The Director was advised that inaccessibility of hazards must be constantly reassessed depending on the children in care.

The fire extinguisher type 2A-10BC must be serviced annually or as often as necessary. Smoke and Carbon Monoxide detectors should be checked and batteries replaced as needed. Changes should be reported to the Department as soon as they occur such as construction, remodeling, telephone number changes and/or if you move from your location.

Mandatory Forms for the children’s files and staff files, requirements for fire drills, earthquake drills and documentation were discussed. Role and responsibilities of being a Mandated Reporter were reviewed. The Director was advised how to access forms and Regulations online at www.ccld.ca.gov. Director was made aware that it is his/her responsibility to know the regulations as well as anyone who assists in providing care.

LPA advised Director that all adults 18 years of age and older providing Care & Supervision and/or have continuous presence in the facility shall adhere to a criminal background clearance with the Department of Justice, FBI and Child Abuse Index Check.

LPA informed Director to log onto web site www.ccld.ca.gov to obtain forms and LIVESCAN application. Records for all children and staff must be maintained for three (3) years after separation from the facility.

The Director was also advised of the requirement to report Unusual Incidents and/or injuries to the parent/guardian and to CCL within the time frame specified by the regulation. Director advised to visit www.shotsforschool.org for immunization information.

The facility was found in compliance per Title 22 regulations, there will be no deficiencies cited today 10/22/2021.

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site inspection by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
