On 8/23/24 at 1:30 pm Licensing Program Analyst (LPA), Claudia Kam made an unannounced required annual visit at above licensed facility. LPA disclosed the purpose of the inspection and was granted entry into the facility.

LPA met with director Alma Guzman and was taken on a guided tour of both indoors and outdoors of the facility. LPA observed Teacher-child ratios, and staff names recorded. All center staff that was present during today’s inspection had fingerprint clearance and are associated to the designated license number. Care and supervision evaluated it is determined that the basic needs and personal rights of children are met and appropriate.

The hours of operation are 6:30 am – 6:00 pm, Monday - Friday. Facility is currently licensed for a capacity of 47. At the initial start of the site visit, there is a total census of 35 children and a total census of 9 staff, director, and cook present. The facility is made up of 2 classrooms. Room A is for children 3- 5 years of age: with 19 children and 4 staff, Room B is for children 2-3 years of age: with 16 children and 5 staff present at time of inspection. LPA observed all posting requirements for operation on the posting board and the following documents were observed LIC 203A-License, LIC 610 A-Emergency Disaster Plan, LIC 9148-Earthquake Preparedness Check List, PUB 394-Notification of parent’s rights poster, LIC 613A- Personal Rights, PUB 269- Child Car Seat Law, Menus, and Activity Schedule.

Furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting, and ventilation were evaluated. Children napping equipment was observed as children were starting their nap and was found to meet regulation requirements at time of inspection. Storage for children's belongings, bathrooms facilities, and isolation areas were inspected. Age-appropriate sinks and toilets were inspected for availability, good repair, water temperature, toilet paper, safety, and sanitation. Page 1 of 4

Classroom A and B have a shared jack and jill bathroom with 4 toddler size toilets and 4 toddler size sinks. A changing table is available in classroom B. LPA observed cubbies with dinosaurs, cars, trains, a quiet corner, dramatic play corner, science corner, craft area, soft tunnels and circle time cushions, the toys were observed to be clean and free of hazards at time of inspection. LPA observed carpets and age-appropriate tables and chairs in each classroom. LPA observed red backpacks for first aid kept in each classroom and a smaller kit at the entry to the facility. First Aid supplies were inventoried and were found to be in compliance with regulation.

Facility provides breakfast, snack and lunch, menus were reviewed at time of inspection. All food items are prepared by the facility. Food and snack items are properly stored. Food preparation area was toured and found in compliance for safety, cleanliness, and proper storage. Drinking water is provided via an osmosis water filter system in each classroom. Water is provided in pitchers with disposable paper cups. Carbon monoxide detectors and smoke detectors are present in the facility in each classroom. LPA confirmed detectors at time of inspection and were determined to be in operable condition. Fire extinguishers were observed as serviced on 02/14/2024. Last fire drill was conducted on 08/13/2024.



At 1:00 PM, LPA observed outdoor play area which consists of a play structure with 2 slides, giant plastic Lincoln logs, giant Legos, stepping stones, play kitchen, Tricycles, ride on toys, outdoor patio chairs and tables, with a large shade structure which covers half of the playground. Outdoor equipment was inspected for safety, cushioning material, good repair, and age-appropriateness. Required shade and fencing were inspected. LPA advised Director that the children need to be always within the direct care and visual supervision by the teachers. Per Director, the children have access to drinking water via water pitchers when in the outdoor area. Play area was inspected for inaccessibility to bodies of water; no bodies of water or hazards observed at time of inspection.

LPA reviewed with the director the LIC 311A, Records to Be Maintained At The Facility, for child’s records, personnel records, administrative records, and documents to be posted. Staff and Children’s Records were reviewed. Criminal Record Clearances were reviewed for adults. LPA was unable to observe current CPR and First Aid Certification for center staff. Per the director all staff files are kept at the district office for the Los Angeles School District who reviews and update staff documents and mandated training. Per director there are 3 lead teachers including herself and 7 teacher aides present.

The following were discussed with the Director:
The fire extinguisher type 2A-10BC must be serviced annually or as often as necessary. Smoke and Carbon Monoxide detectors should be checked, and batteries replaced as needed. Changes should be reported to the Department as soon as they occur such as construction, remodeling, telephone number changes and/or if you move from your location. The Director was also advised of the requirement to report Unusual Incidents and/or injuries to the parent/guardian and to CCLD within the time frame specified by the regulation.

Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms.

Facility representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Center was reviewed for lead compliance and center meets standards for lead. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For childcare center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP).

LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Facility representative was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

The facility was found in compliance per Title 22 regulations, there will be no deficiencies cited today 08/23/2024.

A notice of site visit was given and must remain posted for 30 days.
