An unannounced required annual inspection was conducted on this date by Licensing Program Analyst (LPA) Bardo Baluyot due to the Center has a contract with the Federal government for military families through Child Care Aware of America. There are two children in the pre-school under this contract.

Upon arrival, LPA was greeted and let into the facility by Licensee, June Wu who then guided LPA on a tour of the pre-school room, the kitchen, and of the playground. LPA observed 3 staff with 10 children.

The days and hours of operation are Monday-Friday, 7:00 A.M.- 6:00 P.M.

Posting requirements: All posting requirements were observed on the Parent Board, which is located to the wall directly to the left of the front door. The required postings are: the license, Personal Rights, Child Passenger Safety Law, Notification of Parents Rights, and the snack and lunch menus.

Physical Plant: This Center has only one large room with two toilets in the girls restroom, two toilets in the boys restroom, the Director's office and the kitchen/food preparation area. All areas identified on the Facility Sketch were inspected, including the food preparation area (i.e. the kitchen), storage and refrigeration; rest rooms, equipment, outside play area and overall conditions of facility. Furniture and equipment were inspected for age appropriateness and good repair. Telephone service (there is a land line), heating (there is central heating and air-conditioning and a ceiling fan), lighting (there is overhead lighting and natural lighting comes through the doors and windows) and ventilation were evaluated. Storage for children's belongings and an isolation area with a sink and toilet was inspected. Ill children will remain in the Director's office, in which the Staff restroom is located (there is a toilet and sink.) Drinking water is readily available indoors via water jug. Children have personal water bottles which staff refill using water jug when needed. Children also take their personal water bottles with them during outdoor activity and are also refilled by staff when needed.

Age-appropriate sinks and toilets were inspected for availability, good repair, water temperatures, running water, soap, toilet paper, area safety and sanitation. The Center has air dryers in each restroom. There is a carbon monoxide detector located next to the Parent Board outside of the boys restroom; it was tested and is operable. Licensee provided a fire inspection report conducted on 8/17/21 which states they are current and in compliance with all Fire Safety requirements.

Outdoor equipment was inspected for safety, cushioning material, good repair and age appropriateness. There is foam rubber cushioning under the playground equipment. There is an outside hand washing station for kids to use prior to returning to the classroom. The playground is fenced by brick wall on three sides and the Center on one side. The play area was inspected for hazards and inaccessibility to bodies of water.

Food Service: The food preparation area (i.e. the kitchen) was toured for safety, cleanliness and proper equipment. A review of cleaning and food supply storage areas was made. There is a refrigerator microwave and oven in the kitchen. The refrigerator was measured at 40 degrees Fahrenheit. There is a shelf reserved for staff items in the refrigerator labeled "Staff items." The sink in which the dishes are washed has hot water. The Center provides A.M. snack, Lunch and P.M. snack. Menus, food and snacks were reviewed for availability, quantity and appropriateness to children in care. Breakfast is available upon request from 7:00-8:30, A.M. snack is served at 9:30, Lunch is served at 12:00 and P.M. snack is served at 3:30 P.M. All food is prepared in the kitchen area and then eaten at tables. Per Licensee, social distancing guidelines are observed for meals and table activities. The food was reviewed for availability, quantity and appropriateness to children in care. The food is stored in the freezer and the refrigerator and some cabinets.

Health-Related Services: First Aid supplies were inventoried for quantity and expiration dates. There is one First Aid Kit in the office, one in the storage cabinet outdoors and one in a drawer in the kitchen. A review of medication policy, including administering, labeling, storage, and records was made. There is currently one child on prescription medication. (Epi-Pen Exp 6/2022). Per Licensee, the entire facility is designated as a "nut free" facility.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. Transportation: Per Licensee, this facility does not provide transportation for pre-school children.



An Emergency Disaster Drill log is kept. The last fire drill was conducted 08/5/2021 @ 11:45 A.M. Teacher-child ratios were observed and staff names recorded. Care and supervision was evaluated to determine if the basic needs of children are met appropriately. Sign in and out sheets and procedures were reviewed with Licensee, as was the policy of checking children for illnesses. Per Director, parents are still not allowed inside the facility and hand washing and temperature checks are required for all children prior to entering. Sign in and out sheets were reviewed to ensure that the person who signs the child in and out uses their full legal signature and records the time of the day. Staff Records were reviewed to ensure that a health screening report is on file. Children’s Records were reviewed to ensure that Identification and Emergency form and a medical assessment are on file. Criminal Records Clearance for adults and verification of CPR/First Aid and health preventative practices documentation was reviewed. Personal Rights of children were discussed and observed by LPA. Per Director, there are currently no firearms or weapons on the premises.

File review: Inspection of required forms for the children and staff files was made. As there are 16 children currently enrolled in the pre-school, a random sampling of 3 children's files were reviewed for the Admissions Agreement, LIC 700, LIC 702, LIC 701, LIC 613A. LIC 627, LIC 995A, and the Immunization Records. The children are divided up into 2 pre-school groups (ages 3 and 4) and (ages 4-6). Per Licensee, including herself, there are 3 staff who regularly work in the pre-school. All staff files were reviewed for completeness including but not limited to Criminal Record Clearances for adults, Director Qualifications and verification of CPR/First Aid and health preventive practices.

At this time, the licensee is in compliance with Title 22 of the California Code of Regulations and the Health and Safety Code of California, therefore, there are no citations being issued today.

Upon receipt, Licensee, June Wu posted the Notice of Site Visit. The Notice of Site Visit shall be posted for thirty (30) consecutive days. Failure to maintain posting as required will result in a $100 civil penalty.

An exit interview has been conducted with Licensee, June Wu and a copy of this report has been signed and provided. Appeal Rights were also provided and explained to Licensee.
