On 07/23/2023 at 11:20 AM an unannounced Required – 1 Year Inspection was conducted by Licensing Program Analyst (LPA) Katrina Chicote. Facility is currently licensed for a capacity of 39. LPA was buzzed into facility. Facility is composed of two classrooms: RM-1 (2-3s) and RM-2 (4-5s) and outdoor play area. The program currently operates 6:30 AM to 5:30 PM Monday thru Friday. Upon arrival, LPA did not observe any children in classrooms. Per S5, all children and staff were in outdoor play area. LPA singularly toured facility both indoors and outdoors. LPA observed required postings on Licensing Board in both classrooms. LIC 127 was provided during inspection.

Beginning at 11:29 AM, LPA observed outdoor play area which is accessible within gated area of facility property down alley. LPA announced purpose of visit to Teacher, Olivia Gourian (S2). At the initial start of the site visit, LPA observed 22 children with four staff in outdoor play area. Outdoor equipment was inspected for safety, cushioning material, good repair and age-appropriateness. LPA observed two play areas separated by plastic mesh, RM-1 outdoor play area contained multi-level plastic structure with cushioning material below to absorb fall well maintained. LPA observed age appropriate tables and benches in outdoor play area with toys and art materials available for children for outside play free of loose and sharp parts. LPA observed child size bicycles well maintained available in rear area of play area with concrete flooring. LPA observed same observations in RM-2 play area. Required shade and fencing were inspected. LPA advised that the children need to be within the direct care and supervision, including visual supervision of the teacher(s) at all times. The children have access to water coolers and paper cups during outside play. LPA observed children having free access to drinking water at time of inspection. Play area was inspected for hazards and inaccessibility to bodies of water; no bodies of water or hazards observed.

Beginning at 11:35 AM, furniture and equipment were inspected for age appropriateness and good repair in each classroom. LPA observed surface of classrooms to be linoleum through out classroom with rugs in

learning stations to be safe and clean and child size tables and chairs available for children that are well maintained at time of inspection. LPA observed learning stations for dramatic play, math, and science with toys well maintained free of loose and sharp parts and a library area with soft cushion seating available for quiet play. Telephone service, heating, lighting and ventilation were evaluated. There were cubbies observed as storage for children's belongings. LPA observed standard blue cots stored against wall by bathrooms of each classroom at time of inspection. Per S2, linens are provided by families and taken home weekly for washing. LPA observed cots being laid out not hindering walkways to exits and entrances at time of inspection. Availability of drinking water was reviewed. LPA observed water coolers and cups available for children. Age appropriate sinks and toilets were inspected for availability, good repair, water temperature, toilet paper, area safety and sanitation. LPA observed bathroom available in each classroom that had two toilets and two sinks. LPA observed walk up changing table in RM-1 bathroom.

A first aid kit is kept in teacher closet in each classroom and was inventoried for necessary supplies. At 11:42 AM, LPA tested carbon monoxide detector that is located in RM-2 and observed to be in operable condition at time of inspection. LPA observed smoke detectors and fire extinguishers present at the facility. LPA observed fire extinguishers have been serviced on 06/28/2023. Disaster drill log was available and LPA observed last drill conducted on 07/25/2023. Per Director, they do not administer medications to children at the facility. Per Director, isolation area for sick children is on a designated mat in the classrooms. Hazardous items including poisonous cleaning compounds were stored inaccessible to children.

At 11:45 AM, LPA observed Kitchen area Food and snacks were reviewed for availability, quantity and appropriateness to children in care. Food preparation areas were toured for safety, cleanliness and proper equipment. Per Director, facility provides Breakfast, Lunch, and snack to children. LPA observed current Menus posted in each classroom. During inspection, LPA observed children eating lunch in designated classrooms and trash cans to discard food have tight fitting lids.

Teacher-child ratios were observed and staff names recorded. Care and supervision was evaluated to determine if the basic needs of children are met and appropriate. Sign-in and out sheets and procedures were reviewed. LPA observed all 22 children present signed in at time of inspection. Personal Rights of children were observed by LPA.

Beginning at 12:38 PM, staff and children’s records were reviewed for completeness. Inspection of required forms was made and documented on the LIC 857 for children and the LIC 859 for staff. Criminal Record Clearances were reviewed for adults. At least one staff member has current Pediatric First Aid & CPR at time of inspection. Inspection of required forms made. Staff present did have proof of the AB 1207 Mandated Reporter Training certificates on file. Confidential forms were not reviewed during inspection but were discussed.

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. CCC COMPLETED TESTING AND NO LEAD EXCEEDANCES:
Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test.

Licensee (or facility representative) was reminded that all adults 18 and over in the facility, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed facility. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

The facility was found in compliance per Title 22 regulations, there will be no deficiencies cited today.

Notice of site visit was given and must remain posted for 30 days.
