On Wednesday, March 1, 2023 at 9:36 a.m. Licensing Program Analyst (LPA ) Mayra Rivera conducted an unannounced annual inspection and met with site supervisor Lizbeth Aguirre who guided LPA Mayra Rivera on a tour of the facility.

This is a two-session a.m. and p.m. preschool program that consists of four classrooms (Room 1, Room 2, Room 3 and Room 4) and outdoor play yard. This program serves children ages 3 year to 5 years. This program operates Monday - Friday 8:00 a.m. – 3:45 p.m. Sign in and out takes place at the entrance of the classrooms. LPA Rivera observed 26 signatures and 26 children present during the visit. All areas identified on the facility sketch were toured and inspected both indoors and outdoors.



At approximately 9:45 a.m. LPA Rivera entered classroom 1. LPA Rivera observed 6 preschool children playing and staff #1, 2 and 3 present. LPA Rivera observed age appropriate furniture and equipment to be in good condition, free of sharp, not loose, or pointed parts and age appropriate toys accessible to children. LPA observed cubbies with children’s personal items. LPA observed cleaning solutions stored inside top cabinet making it inaccessible for children to reach. LPA Rivera observed a trash bin with a tight-fitting lid. At 9:49 a.m., the carbon monoxide was tested by LPA Rivera. The carbon monoxide is operable.

At approximately 9:56 a.m. LPA Rivera entered classroom 2. Per site supervisor Lizbeth, the classroom is not being used due to low enrollment. LPA observed age appropriate furniture and equipment to be in good condition, free of sharp, not loose, or pointed parts and age appropriate toys accessible to children and a trash bin with a tight-fitting lid.

At approximately 10:05 a.m. LPA Rivera entered classroom 3. LPA Rivera observed 10 preschool children playing and staff #4 and 5 present. LPA Rivera observed age appropriate furniture and equipment to be in good condition, free of sharp, not loose, or pointed parts and age appropriate toys accessible to children. LPA observed cubbies with children’s personal items. LPA observed cleaning solutions stored inside hand washing sink bottom cabinet with a child proof lock and making it inaccessible for children to open. LPA Rivera observed a trash bin with a tight-fitting lid. At 10:09 a.m., the carbon monoxide was tested by site supervisor Lizbeth. The carbon monoxide is operable.

At approximately 10:13 a.m. LPA Rivera entered classroom 4. LPA Rivera observed 10 preschool children playing and staff #6 and 7 present. LPA Rivera observed age appropriate furniture and equipment to be in good condition, free of sharp, not loose, or pointed parts and age appropriate toys accessible to children. LPA observed cubbies with children’s personal items. LPA observed cleaning solutions stored inside hand washing sink bottom cabinet with a child proof lock and making it inaccessible for children to open. LPA Rivera observed a trash bin with a tight-fitting. At 10:19 a.m. the carbon monoxide was tested by site supervisor Lizbeth. The carbon monoxide is operable

LPA Rivera observed the restrooms to be in good condition with working toilets, sinks, hand soap dispenser and paper towels. LPA observed a total of 7 toilets, 6 urinals and 10 sinks. Ill/isolation area children utilize the blue restroom located by the playground.

At approximately 10:22 a.m. LPA Rivera entered the outdoor playground. Due to the rain, LPA observed the playground to be wet. LPA observed age appropriate play equipment and to be in safe condition, free of sharp, no loose or pointed parts. All areas around or under climbing equipment, are cushioned with granulated rubber mat that absorbs a fall. For outdoor water drinking, water pitcher and disposable cups are available. LPA Rivera observed 3 sheds closed and locked. Sheds are utilized to stores toys and outdoor play equipment. LPA observed the side gates closed and lock with two double latches For adequate shade, LPA observed two hip roof shade structures.

LPA did not observe napping equipment because the program is only half day. For ill/isolation area children utilize the director's office and a mat is be available for children to use to rest if needed. During this visit, facility has no children with prescribed on over the counter medication. Facility has children with food allergies (not severe) and sign is posted in the classrooms.

Heating lighting and ventilation were evaluated by LPA Rivera. LPA observed each classroom to have a wall AC unit and wall heater. LPA observed a screen to cover the wall heaters. LPA observed the current room temperatures to be between 69-70 degrees. LPA observed a total of 6 fire extinguishers located in the classroom and outdoor play area and the valve on the green area on the required 2A10BC fire extinguishers indicating fully charged. The fire extinguishers located in the classroom have been serviced on 3/31/22. The 2-fire extinguisher located in the outdoor play are have been serviced on 9/28/22. LPA observed the first aid kits to be fully stocked with band aids, gauzes, adhesive bandages and antiseptics and located in the classroom’s cabinets. LPA observed the fire/earthquake log and last drill conducted on 10/20/22.

This program provides a.m. and p.m. snacks. The snacks are prepared onsite kitchen. A monthly menu is available and posted in the classrooms. For water drinking LPA observed water dispensers and disposable cups in each classroom. At approximately 10:30 a.m., LPA Rivera observed the food preparation area to be clean and free of litter, rubbish, rodents, and/or any other vermin and storage containers for solid waste, including moveable bins with tight-fitting covers that are kept on, and in good repair. The facility was observed to be free of flies, other insects and rodents.

At approximately 10:50 p.m., LPA observed on the bulletin wall the license, Parents Right's, Personal Rights, LIC 610 Emergency Disaster Plan, Facility Sketch, LIC 9148 Earthquake Checklist, PUB 269 Child Passenger System Poster,monthly Menu and Daily Schedule. At 11:20 a.m., LPA reviewed the children’s roster, children and staff files.

LPA Rivera observed site supervisor Lizbeth Aguirre pediatric First Aid/CPR certification dated January 23, 2022, preventative health and safety certification dated September 8 2002. LPA Rivera observed child abused mandated certification (AB 1207) dated 3/13/22 and immunization against pertussis, and measles, and influenza. Director has been informed mandated reporter training must be completed every 2 years, and is available at www.mandatedreporterca.com

2. Breakfast/Lunch/Snack Menus: Menus are required to be posted one week in advance where it is visible by the child's authorized representative. Menus for the past 30 days must also be available upon request.
3. Current Children’s Roster: Each child day care facility shall maintain a current roster of children who are provided care in the facility. The roster shall include the name, address, and daytime telephone number of the child's parent or guardian, and the name and telephone number of the child's physician. This roster shall be available to the licensing agency upon request.
4.LIC 311A - Records to be Maintained at the Facility: LPAs advised that forms, regulations and quarterly updates can be accessed on the Child Care Licensing website at: www.ccld.ca.gov.
5. Pediatric First Aid and CPR: At least one person trained in Pediatric First Aid and CPR must be present.
6. Designated Staff: The name of the childcare center director or fully qualified teacher(s) designated to act in the director's absence must be on file.
7. Qualifications: Educational background, training, and/or experience for each staff present must be available for review.
8. Immunization Requirements: Commencing September 1, 2016, a person shall not be employed or volunteer at a day care center if he or she has not been immunized against influenza, pertussis, and measles
9. Children’s Records: must be available for review; including but not limited to, the following: Name, address and telephone number of the child's authorized representative and of relatives or others who can assume responsibility for the child if the authorized representative cannot be reached when necessary.

Site Supervisor Lizbeth was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

During this visit, no citations were given. A notice of site visit was given and must remain posted for 30 days.
