At 12:15 PM, Licensing Program Analyst (LPA) Cynthia Reyes, conducted an announced Pre-Licensing of a Toddler Option Inspection to measure this facility for capacity determination. Licensee also request to decrease the preschool capacity from 120 to 108. LPA met with Michelle Gallo, Director, who provided LPA with an indoor and outdoor tour of the facility. The applicant is requesting a capacity of 12 toddler children. The main entrance to the facility is located in the front of the day care. LPA did not observe any children on site at this time due to being released early because of the local fires in the area and the bad air quality.

This is a proposed toddler option attached to the licensed Preschool Program. The toddler option consist of one toddler class room (#12), and outside around the corner is a bathroom with 3 sinks (45 children) and 2 toilets (30 children )and a changing table with in arms reach of a sink. This program will serve preschool children ages 2 to 5 and toddlers are 18 to 36 months. The preschool & Toddler option program will operate a full day session from 7:00 AM. to 6:00 PM. from Monday to Friday.

All areas identified on the facility sketch were toured both indoors and outdoors; measurements were taken of the toddler playground allowing for a capacity of 40 toddlers. Sign-In and Sign-Out sheets are located at the entrance of the main office. During Covid-19 sign in/out is located outside by the front entrance. No parents are allowed inside the center at this time. Health checks which include temperature being taken of children and parents will be conducted prior to parents signing children in daily. No transportation is being conducted by the facility.

During the tour of all class rooms, Toddler and Preschool, Furniture and equipment were inspected for age appropriateness and good repair. LPA observed napping mats are located in a storage cabinet in each class room. There is accessible drinking water readily available, Sippy cups & water bottles will be labeled.

Toys were observed to be clean and safe and age appropriate. Telephone service, heating, lighting and ventilation were evaluated. Toddlers each have their own cubby to store their belongings. Child own Linens will be taken home weekly on Fridays by child’s authorized representative for laundering. LPA observed the facility has a carbon monoxide detector and smoke detector in each classroom. There is a serviced fire extinguisher available in each class room. Disinfectants, cleaning solutions, and other items that are dangerous to children, will be inaccessible at all times.

This program will provide AM/PM snack and child may bring their own lunch or they have the option of buying lunch from the district. During Covid-19, parents will bring the children own lunch and AM/PM snack. A monthly snack menu is available and posted. LPA advised the Applicant to label and date all food, dishes and bottles that are brought from home for proper storage.

All storage containers for solid waste, including moveable bins shall have tight-fitting covers that are kept on, and in good repair. Trash cans used to discard food have tight fitting lids. First Aid kits are available in each classroom and were observed to contain the required first-aid supplies.

Outdoor play equipment was inspected for safety, free of sharp, loose pointed parts, good repair and age appropriateness. The surface of the outdoor activity area was observed to be maintained in a safe condition, and free of hazards. All areas around or under high climbing equipment, swings, slides, and similar equipment are cushioned with material that absorbs a fall. Required shade and fencing were inspected. Drinking water will be available outdoors via pitchers of water with cups. The playground was inspected to ensure no bodies of water or hazards which were not observed.

A small office in the main office will be used as an ill/isolation area for toddler and preschool children. LPA observed an ill child cot/mat available. Ill children will use the restroom located in the same small office.

Items discussed during this Inspection:
Current Children’s Roster LIC 9040: Each child day care facility shall maintain a current roster of children who are provided care in the facility. The roster shall include the name, address, and daytime telephone number of the child's parent or guardian, and the name and telephone number of the child's physician. This children roster must be updated and available to the licensing agency upon request. Continue on page 3

Pediatric First Aid and CPR: At least one person in the facility, along with the Director/Administrator must be trained in Pediatric First Aid and CPR and present in the facility.
Qualifications and staff files: Educational background, training, and/or experience and all required documents by the department for each staff present must be available for review.
Children’s Records: All required licensing documents must be available for review; including but not limited to, the following: Name, address and telephone number of the child's authorized representative and of relatives or others who can assume responsibility for the child if the authorized representative cannot be reached when necessary. Director advised to visit www.shotsforschool.org for immunization information.
Immunization Requirements: Commencing September 1, 2016, a person shall not be employed or
volunteer at a day care center if he or she has not been immunized against influenza, pertussis, & measles.
Criminal background clearance: LPA advised Director that all adults 18 years of age and older providing Care & Supervision and/or have continuous presence in the facility shall adhere to a criminal background clearance with the Department of Justice, FBI and Child Abuse Index Check.
Reporting Requirements Section 101212: Changes at the facility must be reported to the Department as soon as they occur such as construction, remodeling, telephone number changes and/or if you move from your location. The Director was also advised of the requirement to report Unusual Incidents and/or injuries to the parent/guardian and to CCL within the time frame specified by the regulation.

Incidental Medical Services (IMS): This facility plans to provide Incidental Medical Services – IMS. For IMS information, see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. A Plan of Operation that includes IMS must be submitted to the Department. AB1207: The following was discussed: As of January 1, 2018, Health & Safety Code 1596.8662 requires all licensed providers, applicants, directors and employees to complete training as specified on their mandated reporter duties and to renew their training every two years. New employees shall have 90 days from date of employment to complete training as required. The training may be met by using the Department’s Office of Child Abuse Prevention (OCAP) online training modules at: http://www.mandatedreporterca.com/.

The preschool classes licensed and used will be rooms 10, 11. 13, 14 and the Cafeteria/Multipurpose room.

LPA advised the Applicant to access forms, regulations and quarterly updates on the Child Care Licensing website at: www.ccld.ca.gov.

LPA observed the parent board located at the main front entrance office for the preschool class rooms to have all the required licensing forms accessible for parents to view, which included the Parent’s Rights Poster (with complaint hot line phone number).

Based on the LPA observation, the following corrections need to be corrected before a license can be given.
No Corrections are needed.

A fire clearance was granted on 08/11/2020 for a capacity of 120, for 108 preschool and 12 toddlers.

Based on LPA measurements taken on this date, the capacity for the preschool program will decrease to 108 and the 12 toddlers requested will be granted based on measurements for room 12 allowing capacity of 22.

Exit interview was conducted in person with Michelle Gallo, Director. A copy of this report (LIC 809) and Appeal Rights (LIC 9058) were discussed, signed and printed and given to applicant on this date. Consultation regarding the licensing state laws and regulations, were conducted through out this inspection.

All precautionary measures regarding Covid-19 were taken by LPA and facility staff on this date and discussed with the applicant. Documents regarding Covid-19 will be emailed to the applicant.


******During this Inspection of the facility, LPA recommended and advised the Director that a gate should be placed at both ends of the entrance to the front of the school, to prevent a child from wondering out of the facility on to a high traffic street and to also prevent outsiders from walking in freely to the child day care facility.
