25 | Licensing Program Analyst (LPA) Lissete Gonzalez conducted an unannounced case management inspection to follow up on an incident that was reported to the Department on 06/07/2019. Upon arrival, LPA met with Licensee, Yesmin Escalante, who provided LPA with a tour of the facility inside and outside. The Licensee, Assistant 1 (A1), Assistant 2 (A2) and 8 children were present.
On 06/07/2019, an incident was reported to the department where a child was suffering from a fever with a temperature that was documented at 11:47 a.m. that read 99.1. The child’s authorized representative was sent a text message at 11:49 a.m. by the Licensee regarding the fever. At 2:00 p.m., the child began to have convulsions at which time paramedics were called and the child was transported to the hospital. The facility reported this incident to the Department within the required 24 hours. During this inspection LPA interviewed Licensee, Assistant 1, Assistant 2, and reviewed the parent handbook for the policy regarding ill children. LPA observed the indoor napping area and the child’s “Health Daily Checklist” chart that was initiated for all children in care as of 6/10/2019. LPA obtained and reviewed documentation. Based on the information obtained and LPA observations, LPA determined further information is needed to make a determination regarding the incident.
