On 3/17/2025 at 2:00 PM, Licensing Program Analysts (LPAs) Carolyn Tuba and Diana Ortiz conducted an unannounced required inspection visit. LPAs met with Assistant Director, Alicia Rodriguez. A COVID risk assessment was conducted. At approximately 2:30 PM Director, Adetayo Akiwowo arrived at the facility. Director then guided LPAs on a tour of the facility indoors and outdoors. This is a preschool age program (2-5 years old) licensed for 37 children, which operates Monday through Friday from 6:30AM to 7:00 PM. Per the Assistant Director, there are 54 children enrolled who come at various schedules. LPAs obtained a census of 24 children with 3 staff present and with additional adults (therapists).

The facility is located inside of the Alexander Hughes Community Center. The following classroom was inspected: ABC classroom, which is one entire room that accommodates the combined ages of children. There is a quiet area located off of the main room. The isolation room used is the Director’s office, which has a staff bathroom right off the office side door. There is an employee lounge located off of the main lobby near the Director's office. Teacher-child ratios were observed to be in accordance with Title 22 Regulations. Staff names were recorded. All children were observed to be under supervision, including visual supervision. Licensed facility is within the conditions, limitations, and capacity specified on the license.

The classroom furniture and equipment were inspected for good repair, free of sharp, loose, or pointed parts. All indoor classroom was inspected to ensure that the floors have a surface that is safe and clean. There are 3 child size toilets and 3 child size sinks. All toilets and washing facilities are in safe and sanitary operating conditions.

The facility provides breakfast, lunch and PM snack and lunch is provided by parents. The facility is not part of a food program. Extra food is available if parents forget to bring lunch. Extra snacks are provided if children are hungry at the end of the evening since the facility closes at 7 pm. The meals are prepared in the

kitchen area with extra space utilized as a pantry. A 30-day snack menu is posted right outside of the main classroom for parents to observe. Facility will update the menu with any changes.

LPAs observed smoke detector. Carbon monoxide detectors were found to be operating during the visit. Assistant Director stated that the Fire Marshalls last conducted a visit in September 2024 and serviced fire extinguishers. Emergency disaster drill was last conducted and documented in February 2025. There are First aid kits available, and supplies were inventoried and found to be complete including surgical scissors, tweezers and emergency booklet.

Outdoor playground equipment is in safe condition, free of sharp, loose, or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards, however LPAs observed blacktop has a large crack that extends 25 feet located near the classroom door that can pose a danger to the children in care. (Tech Advisory Issued) All areas around or under climbing equipment, slides, and similar equipment are cushioned with a soft foam material that absorbs falls. LPAs reminded that staff rake through the sand areas daily and inspect for animal feces and or any unsafe objects. There are several trees that provide adequate shade in the play yard. There are tricycles stored in the shed near the play yard. Area is gated with high iron gates and there is a side entrance that parents may use, that require a code to enter. There is a blacktop area and a tri-level basketball structure for children to play. During the visit Assistant Director provided an Unusual Incident Report (UIR) pertaining to new equipment playground upgrade being conducted March 31- April 11, 2025. LPAs advised that a Case Management visit will be needed after the remodel.

Sign-in and out is done electronically with a QR code that is scanned by the authorized individual using their cell phone called SmartCare. Assistant Director stated that a report can be printed if needed. Parents and authorized individuals have an app and are able to sign their child in and out each day. A paper sign-in and out sheet is used if a parent forgets to bring in their phone or if the system does not work.

LPAs reviewed staff records, and were found to be complete. Clearance for adults and verification of CPR/First Aid and health preventative practices documentation was reviewed. Children’s records were reviewed to ensure that Identification and Emergency form and a medical assessment are on file. The Director and staff currently have immunization documentation.

Per Director medication is kept in a container in the kitchen, in a locked cupboard and an IMS plan is available in the child’s file. LPAs observed current medication and reviewed the paperwork. Director was reminded that should a child leave the facility or medication expires, it should be returned to the parent.

AB1207 Mandated Child Abuse Reporting – On or before March 30, 2018, any person who works in a childcare facility shall complete the training and renew the training every 2 years. Director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of “medication and equipment/supplies, and reviewed children’s, personnel, and administrative records.
