25 | Licensing Program Analyst (LPA) Alanna Gontarek conducted a case management- other, due the Licensee requesting to add a converted garage and patio space to the daycare areas. LPA met with Licensee, Carmen Ore, who gave a tour of the facility indoors and outdoors. Census was taken. There was 1 child and 1 staff present upon arrival. Licensee stated she will be using the converted garage and small patio for the daycare.
LPA obtained pictures on 4/8/2021 of the converted garage space, which includes a bathroom, and small kitchen., and a written statement from the Licensee stating she will provide 100% supervision of children while eating and sleeping in the approved converted garage/ Dwelling Unit.
On 7/7/2021, LPA Gontarek received a new Certificate of Occupancy City of La Puente Building and Safety. Building Permit # 028086; Occupancy Group: R-3; Description: 400 sq. ft. Attached Garage Conversion- Habitable Space, certified on 7/1/2021. The converted garage/dwelling unit’s building address is the same as the owner, Carmen Ore’s, property address: 437 Glenshaw Avenue La Puente, CA. 91744.
On 7/28/2021, the Licensing Department received the Fire Safety Inspection Clearance from LA County Fire Department stating the garage conversion was approved on 7/27/2021, as a “Habitable space in the use as a childcare space.”
During today's visit, at 9:30 a.m., LPA observed, and assessed, the converted garage and patio space to be safe and have no hazards. Pictures of patio space were taken during today's visit. Per Licensee, will install shade. Licensee agrees to notify department if in the future there are any changes to the permit issued by the city. Declaration was obtained during today’s visit.
There were no deficiencies cited at this time. Licensee is in compliance with Title 22 regulations.
An exit interview was conducted with Licensee, Carmen Ore. A copy of this report (LIC 809), Appeal Rights and Notice of Site Visit were discussed and provided to the Licensee during today’s visit. |
