On 09/21/2023 at 1:25 PM an unannounced Required – 1 Year Inspection was conducted by Licensing Program Analyst (LPA) Katrina Chicote. Licensing staff announced purpose of visit and met with Ernestine Lara, Principal, who granted entry to facility and provided tour of the facility both indoors and outdoors. Facility is currently licensed for a capacity of 175, per Principal they currently have 80 preschoolers enrolled. The program currently operates two programs General Ed Monday thru Friday from 7:00 AM to 5:30 PM and part time Collaborative Program Monday thru Friday from 8:00 AM to 12:30 PM. LPA observed required licensing postings at facility reception area and posted on a board by each classroom.

At the initial start of the site visit, there is a total census of 48 preschoolers and a total census of 19 staff members present. The facility is made up of seven classrooms, per Principal they are currently only utilizing five of them (RMs 1, 2, 4, 5, and 6) and RMs 3 and 7 are closed at this time. LPA observed RMs 3 an 7 to be vacant with no children or staff at time of inspection. Per Principal, Collaborative Program are in RMs 4 & 5. LPA arrived during facilities nap time and observed preschoolers napping in each classroom using standard blue cots that are laid out without blocking entry ways or access to exits and entrances.

Beginning at 1:30 PM, furniture and equipment were inspected for age appropriateness and good repair in each classroom. LPA observed surface of classrooms to be safe and clean, and child size tables and chairs available for children well maintained. LPA observed learning stations for dramatic play, math, and science with toys well maintained free of loose and sharp parts and a library/cozy area available for quiet play. Telephone service, heating, lighting and ventilation were evaluated. There were cubbies observed as storage for children's belongings. Per Principal, linens are provided by the facility and washed weekly by laundry service. Availability of drinking water was reviewed. LPA observed water fountains available for children as well as personal water bottles. Age appropriate sinks and toilets were inspected for availability, good repair, water temperature, toilet paper, area safety and sanitation. LPA observed each classroom to have bathroom available through their classroom and additional facilities outside of classroom.

Snack/lunch menus were reviewed and observed posted at time of inspection. All meals and snacks are provided by Los Angeles Unified School District which is delivered to facility from nearby elementary school campus. Per Principal, children are provided with AM Breakfast, lunch, and PM snack. Trash cans used to discard food have tight fitting lids.

A first aid kit is kept in a red backpack in each classroom. At 1:35 PM, carbon monoxide detectors were tested and observed operable at time of inspection. LPA observed smoke detectors and fire extinguishers present in the facility. LPA observed disaster drill log available, last drill conducted on 09/20/2023. Disinfectants, cleaning solutions, medication and other items that are dangerous to children are inaccessible Per Principal these items are not stored in classrooms. At 1:40 PM, LPA observed medicine that is stored in RM-4 for C1 to have required documents and to be stored per Title 22 Regulation requirements. LPA observed isolation for sick children is in Nurse's Office, LPA observed a bed and separate bathroom available for sick children.

Beginning at 2:00 PM, outdoor equipment was inspected for safety, cushioning material, good repair and age-appropriateness. LPA observed various benches with shade available, two play structures that has cushioning material to absorb fall that is well maintained, basketball court, bike path, and a nature exploration area available for children. LPA observed bench area under shad with age appropriate toys free of loose and sharp parts that are in outdoor storage that do not have lids. Required shade and fencing were inspected. LPA advised that the children need to be within the direct care and supervision, including visual supervision of the teacher(s) at all times. The children have access to their water fountains for outside play. Play area was inspected for hazards and inaccessibility to bodies of water; no bodies of water or hazards observed.

Teacher-child ratios were observed and staff names recorded. Care and supervision was evaluated to determine if the basic needs of children are met and appropriate. Sign-in and out sheets and procedures were reviewed. LPA observed all children present signed in at time of inspection. Personal Rights of children were observed by LPA.

Beginning at 2:30 PM, staff and children’s records were reviewed. Children’s Records were reviewed for completeness. Inspection of required forms was made and documented on the LIC 857. The review of Staff records was documented on the LIC 859.

Staff files reviewed did have, proof of immunization records complete, proof of current AB 1207 Mandated Reporter Training certificate on file, and verified proof of education credentials. LPA observed Pediatric First Aid/CPR expires on 06/2025 for Staff #5. LPA did not review confidential staff files during the inspection due to the staff confidential files being stored at the Administrative Office. All staff and employees have obtained a criminal record clearance or criminal record exemption as a condition of employment with Los Angeles Unified School District.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. CCC COMPLETED TESTING AND HAS REMEDIATED LEAD EXCEEDANCES.

The facility was found in compliance per Title 22 regulations, there will be no deficiencies cited for this visit.

A notice of site visit was given and must remain posted for 30 days.
