Licensing Program Analyst (LPA) Rita Ramos conducted an unannounced case management inspection to the above facility to conduct a second follow-up visit for incidents that occurred on 10/18/18 and on 04/04/18. LPA met with Celestine Pearman, Principal, who guided analyst on a tour of the facility. There were 121 children present with 23 staff upon arrival.

LPA conducted interviews and obtained documentation during this visit.

The incident that occurred on 10/18/18, was reported to the Department on 10/22/018, via telephone. The facility did not report the incident within the required 24 hours and was cited on 11/16/18 for not reporting timely. The incident that occurred on 04/04/18 was reported timely on 04/05/18.

Information reported to the Department for the 10/18/18 incident indicated that Child #1's personal rights may have been violated by Staff #1. The 04/04/18 incident indicates that Child #2's personal rights may have been violated.

Based upon information received from the interviews conducted it was determined that there there is not enough evidence to determine whether or not the personal rights of Child #1 and Child #2 were violated.

There were no deficiencies cited during today’s inspection

The Notice of Site Visit (LIC 9213) – must remain posted for
30 days during the hours of operation after each site visit by a licensing representative. Exit interview was conducted with Celestine Pearman, Principal including, but not limited to Provider Rights, Appeal Procedures
