On 12/06/2023 at 12:35 PM Licensing Program Analyst (LPA) Katrina Chicote conducted an Unannounced Case Management - Incident inspection to follow up on an incident that was reported to The Department on 10/13/2023. LPA announced purpose of visit and was granted entry to facility. LPA initially met with Head Teacher, Mea Smith, during inspection due to Principal, Viviana Ledezma, not being present.

The incident that occurred on 10/12/2023 was reported to the Department within 24 hours of incident and written report submitted within seven days via email.

An unusual incident report was made to The Department regarding an incident that occurred involving a child making an allegation towards a staff member. During inspection, LPA conducted interviews however LPA was unable to interview staff who reported incident or Principal. LPA interviewed three staff present during inspection and one staff stated they were not aware of any incident, another stating they had minimal knowledge of incident, and another staff member, named on report, disclosing more details in regards to incident. Child named in report is still attending facility and LPA interviewed child and was not able to qualify at time of inspection. No disclosures and corroborating information were provided in regards to incident and staff with knowledge of incident states it was investigated by Agency which included interviews of staff and children and investigation has since been closed with unsubstantiated finding. Staff confirm they did receive training a couple months ago in regards to handling challenging behaviors with children, confirming information on report in regards to actions taken by facility.

Based on the information obtained, there are no citations in regards to this incident.

A notice of site visit was given and must remain posted for 30 days.

Exit interview was conducted and report was reviewed with the Facility Representative, Brenda Ventura.
