25 | On 05/08/2024 at 2:00 PM Licensing Program Analysts (LPAs) Katrina Chicote and Claudia Kam conducted an Unannounced Case Management inspection to follow up on incidents that were reported to The Department on various dates. Upon arrival, LPAs announced purpose of inspection and met with Facility Representative (FR), Viviana Ledezma, Principal. Census was taken.
The facility reported incidents to The Department within the required 24 hours and submitted a written report within seven days of incidents per Title 22 Regulation requirements.
On 04/25/2024 and 04/17/2024, incidents were reported to The Department where a parent made an allegation towards a staff member violating her child's personal rights. During this inspection, LPAs interviewed Principal, Principal states that per LAUSD guidelines they have conducted their own internal investigation which includes reporting to LAPD and CAHL. Principal states she attempted to report it to both Agencies but due to lack of information reports were only taken as a consultation at this time. FR states parent of child did not provide any staff names for incidents reported. FR states that child is no longer enrolled at the school and has since not returned. LPAs notified FR that this incident is being investigated by Licensing's IB Department for further information and LPAs obtained Facility Roster during inspection.
On 04/05/2024, incident was reported to The Department regarding physical plant updates. Per report, flooring in main lobby was going to be updated which will affect how families access facility. Report states that construction will conclude on 04/12/2024. LPAs arrived at facility and did not observe any construction and floors to be updated. Per FR, floor construction was concluded on time.
The facility was found in compliance per Title 22 regulations, there will be no deficiencies cited today, 05/08/2024.
A notice of site visit was given and must remain posted for 30 days.
Exit interview was conducted and report was reviewed with the Facility Representative, Viviana Ledezma.
