On 07/11/2024 at 1:40 PM an Uunannounced Annual/Random Inspection was conducted by Licensing Program Analysts (LPAs) Katrina Chicote and Peter Bishop. LPAs announced purpose of visit and initially met with Office Administrator, who allowed entry to facility. Facility is currently licensed for a capacity of 175 and per Facility Roster have 153 enrolled. The program currently operates Monday thru Friday from 7:00 AM to 5:30 PM. LPAs were taken on a guided tour of the facility of both indoors and outdoors by Facility Representative (FR), Mea Smith. Required postings were observed. LIC 125 was provided.

The facility is made up of seven classrooms: RM-1, RM-2, RM-3, RM-4, RM-5, RM-6, and RM-7. At the initial start of the site visit, LPAs observed that children were transitioning out of nap time. LPAs observed RM-1 to have eight children with three staff, RM-2 to have six children with three staff, RM-3 to have five children with three staff, RM-4 with five staff with three staff, RM-5 with seven children with three staff, RM-6 with 14 children with three staff, RM-7 with three kids with three staff. LPAs observed total census of 48 children and a total census of 24 staff present at time of inspection.

Beginning at 2:00 PM, furniture and equipment were inspected for age appropriateness and good repair in each classroom. LPAs observed surface of classrooms to be safe and clean, and child size tables and chairs available for children with rounded edges and well maintained. LPAs observed learning stations for dramatic play, math, and science with a variety of age appropriate toys maintained free of loose and sharp parts and a library area available for quiet play. Telephone service, heating, lighting and ventilation were evaluated. There were cubbies observed as storage for children's belongings. Cots were inspected and observed stored in a closet. LPAs observed cots laid out for nap time in a way that does not hinder walkways or entrances. Per FR linens are provided by the facility and washed by weekly laundry service. Availability of drinking water was reviewed. LPAs observed drinking fountains available for children and children's personal water bottles. Age appropriate sinks and toilets were inspected for availability, good repair, water temperature, toilet paper, area safety and sanitation. and RM-6 and RM-7, with three toilets and two sinks each; RM-3 had it's own bathroom with two toilets and two sinks. LPAs observed additional sinks that were operable at time of inspection available in each classroom. Per FR, only LAUSD approved disinfectants are present at the facility which are not stored inside the classrooms.

LPAs observed a first aid kit is kept in each classroom and inventoried for necessary supplies. LPAs tested carbon monoxide detectors, located in Kitchen, and observed to be in operable condition at time of inspection. LPAs observed smoke detectors and fire extinguishers are present in the facility. LPAs discussed requirement to have fire extinguishers serviced annually and to conduct regular checks of smoke and carbon monoxide detectors to ensure they are in operable condition. Disaster drill log was available and last drill conducted was on 06/25/2024. Isolation area for sick children is in Nurse's Office. Per FR, they have no children requiring medications present at time of inspection. FR states any medicines needed for children is stored in Nurse's Office.

Snack/lunch menus were reviewed and observed posted. Facility provides Breakfast, Lunch, and PM Snack. Per FR, food is delivered from nearby elementary school campus and distributed at the facility by Staff. Food preparation areas were toured for safety, cleanliness and proper equipment. Trash cans used to discard food have tight fitting lids.

Beginning at 2:30 PM, outdoor equipment was inspected for safety, cushioning material, good repair and age-appropriateness. LPAs observed surface of outdoor play areas to be safe and maintained for activities planned. LPAs observed a variety of age appropriate toys available for outdoor play such as manipulatives and bicycles. LPAs observed an age appropriate multi-level play structure with cushioning material below to absorb fall well maintained at time of inspection. Required shade and fencing were inspected. LPAs observed shaded area with seating available for children. LPAs advised that the children need to be within the direct care and supervision, including visual supervision of the teacher(s) at all times. The children have access to drinking fountains and their water bottles during outside play. Play area was inspected for hazards and inaccessibility to bodies of water; no bodies of water or hazards observed.

Teacher-child ratios were observed and staff names recorded. Care and supervision was evaluated to determine if the basic needs of children are met and appropriate. Sign-in and out sheets and procedures were reviewed and discussed. Personal Rights of children were observed by LPAs.

Beginning at 3:10 PM, staff and children’s records were reviewed for completeness. Inspection of required forms was made and documented on the LIC 857 for children and the LIC 859 for staff. LPA did not review confidential staff files during the inspection due to the staff confidential files being stored at the administrative office. CPR card expires on 09/2025 for Staff #2. 6 of 6 staff files reviewed did have proof of current AB1207 Mandated Reporter Training certificate on file. All staff and employees have obtained a criminal record clearance or criminal record exemption as a condition of employment with the Los Angeles Unified School District (LAUSD).

The following were discussed:
LPA advised of the requirement to report Unusual Incidents and/or injuries to the parent/guardian and to CCL within the time frame specified by the regulation, this includes any changes as soon as they occur such as construction, remodeling, telephone number changes and/or if you move from your location.

CCC COMPLETED TESTING AND REMEDIATED ALEs:
Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test.

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. MyChildCarePlan.org – Centers and Family Child Care Homes Licensee [or facility representative] was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

The facility was found in compliance per Title 22 regulations, there will be no deficiencies cited today only advisories given, 07/11/2024.

A notice of site visit was given and must remain posted for 30 days.
