On 3/22/2024 at 11:00 AM Licensing Program Analyst (LPA) Katrina Chicote conducted an Unannounced Case Management inspection to follow up on multiple incidents that were reported to The Department. The facility reported these incidents to the Department within the required 24 hours and submitted a written report within seven days per Title Regulation requirements. Upon arrival, LPA met with Facility Representative Mea Smith, Head Teacher, who provided LPA a tour of the facility. Census was taken. Principal was not available during today's inspection due to a LAUSD District wide Principal training.

On 02/22/2024, an incident was reported to The Department via telephone in regards to an allegation towards a staff member of a child's personal rights being violated.

On 02/29/2024, an incident was reported to The Department via telephone in regards to additional allegations made towards same staff member of a child's personal rights being violated.

During today's inspection, LPA interviewed staff in classroom and attempted interviews of children. LPA was not able to qualify children for interviews. Staff interviews did not provide any corroborating information to indicate incident occurred or is valid. Staff interviews all consistently state that they work together and are not alone with the children and did not observe the incidents described in reports submitted. Per report and staff interviews, the facility has conducted it's own internal investigation and finding was provided though staff are unsure of the results of finding, though they believe the finding was found to be invalid. Due to insufficient information being available at this time, a follow up with Principal on results of internal investigation will be conducted by LPA.

The facility was found in compliance per Title 22 regulations, there will be no deficiencies cited today, 03/22/2024.

A notice of site visit was given and must remain posted for 30 days.

Exit interview was conducted and report was reviewed with the Facility Representative, Mea Smith.
