Licensing Program Analysts (LPAs) Rita Ramos and Alicia Mooberry conducted an unannounced required/annual inspection. LPAs met with Ana Vidal, Principal, who guided analysts on a tour of the facility. This is a preschool program which consists of 7 classrooms; Room 1 through Room 7. Facility operation hours are Monday through Friday from 7:00 AM to 5:30 PM.

All areas identified on this report were inspected. Upon arrival, the following staff were present during this inspection: Room 1: Staff #1, #2 and #3 with 20 preschoolers; Room 2: Staff #4, #5, #6, #7 with 20 preschoolers; Room 3: Staff #8, #9, #10, and #11 with 22 preschoolers. Room 4: Staff #12, #13, #14, and #15 with 20 preschoolers, Room 5: Staff #16, #17, and #18 with 19 preschoolers; Room 6: Staff #19, #20, #21, and #22 with 19 preschoolers, and Room 7: Staff #23, #24, #25, #26, and #27 with 22 preschoolers. The following was observed during the tour of facility:

Furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting and ventilation were evaluated. Children have their own cubby to store their belongings. Linens are washed weekly by a laundry service provided by the school district. Napping equipment (cots) were observed in each classroom. Per Principal, the isolation area is located in the nurse's office. Age appropriate sinks and toilets were inspected for availability and good repair in all restrooms. LPAs observed that facility does have a carbon monoxide detector. General sanitation was observed. Availability of indoor drinking water was observed in classrooms.


Disinfectants, cleaning solutions, medication and other items that are dangerous to children, were inaccessible to children. Per Principal, there are no poisons stored at the facility.

This facility does not have a kitchen. All meals and snacks are delivered in a cart from the elementary school next to the facility. Trash cans used to discard food have tight fitting lids. ---------Page 1 of 3

Outdoor playground equipment is in safe condition, free of sharp, loose or pointed parts. LPAs advised the Principal to ensure that the children need to be within the direct care and supervision, including visual supervision, of the teacher(s) and supervising staff at all times. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All areas around or under high climbing equipment, slides, and similar equipment are cushioned with material that absorbs a fall. There is adequate shade in the play yard. Availability of outdoor drinking water was observed via 2 faucets, however, one of the faucets was not functioning. The Principal provided LPAs with a copy of the trouble call log that was made to the school district on 09/04/19.

All individuals present have obtained a criminal record clearance or criminal record exemption as a condition of employment with the Los Angeles Unified School District. There is at least one person trained in CPR and Pediatric First Aid present during this inspection.

Children’s Records were reviewed for completeness; Inspection of required forms was made. LPAs issued the Children’s Record Review (LIC 857) to the Principal during this inspection. In review of children’s records, files contain information including, but not limited to the following: Name, address and telephone number of the child's authorized representative and of relatives or others who can assume responsibility for the child if the authorized representative cannot be reached when necessary.

LPAs also issued the Review of Staff records (LIC 859) to the Principal during this inspection. Staff present have the required immunization records on file. The LIC 859 documents the staff files that were reviewed during this inspection.

Children's roster was reviewed and is current. Sign-in and Sign-out sheets were reviewed. Children present were signed-in. Disaster drill log was available, last drill was conducted on 08/28/19.

Menus are posted one month in advance where it is visible by the child's authorized representative. Menus for the past 30 days are available upon request. Snacks were reviewed for availability, quantity and appropriateness to children in care. Preschoolers are provided with a breakfast, lunch, and snack. All meals and snacks are provided by the Los Angeles Unified School District.

First Aid supplies were observed in the classroom in a backpack and in kit-boxes. According to the Principal, medication is only administered to a child when accompanied with a doctor's note and is stored in the nurse's office. Upon inspecting the medication, LPAs observed that Child #4 had an expired medication. The Office Manager provided LPAs with a copy of a letter provided to the parent of Child #4 on 07/17/19 requesting to replenish the medication. LPAs advised the Office Manager and the Principal, that if medication is going to be stored and administered at the facility, all medication needs to be dated. The Office Manager called the parent of Child #4 during the inspection to have the medication replenished.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. LPAs advised the Principal to access forms, regulations and quarterly updates on the Child Care Licensing website at: www.ccld.ca.gov.

There were no deficiencies cited during today’s inspection.

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Exit interview was conducted Elizabeth Okoro, Head Teacher, including, but not limited to Provider Rights, Appeal Procedures and Agencies Consultative Role.
