INSPECTION CONDUCTED IN SPANISH
Licensing Program Analyst (LPA) Denise Gibbs conducted an unannounced Case Management inspection to the above facility to obtain information from licensee on 11/1/19 at 9:00 AM. LPA initially met with Carolina Carasco, Assistant who informed LPA that Licensee would be back soon and guided LPA on a tour of the facility. There were 2 children present upon arrival. At 9:20 AM Licensee Leonor Sanabria arrived.

LPA informed licensee that a form was received by mailing in the Regional Office stating that she would like to close her license. LPA is aware that licensee is still operating. Licensee stated that she did not know what the form said due to it being in English. Licensee would like LPA to disregard the form because she is still operating and does not want her license closed.

LPA inquired about immunization's for Lenor Gastelum, (licensee's mother) that was cited during an Annual Inspection on 8/21/19. Licensee provided information that due to personal reasons her mother will not be working or residing at the facility for up to a year.

LPA assessed the overall heath and safety of children in care, based on observation and interview, at this time, the licensee is in compliance with California Code of Regulations Title 22. No deficiencies cited.

The Notice of Site Visit (LIC 9213)must remain posted for 30 days during the hours of operation after each site visit made by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
