Visit conducted in English and Spanish using Translation #14629
Licensing Program Analyst (LPA) Warren Birks conducted an unannounced Case Management Incident inspection. This inspection is regarding incident/injury that took place on February 18, 2022. LPA initially met with Assistant Carolina Carasco who was caring for 10 children. Licensee Leonor Sanabria arrived at approximately 2:55pm from conducting a pick up.

Licensee Sabrina provided information and assistance for the inspection. At 2:25pm LPA observed Licensee's assistant caring for 10 children (7 preschool, 2 school age and 1 infant) with no other staff. LPA informed Licensee via translation assistance (from LPA R. Navarro over the phone) that the purpose of the visit is to inquire about an injury that took place, In addition, LPA informed Licensee via translation that the facility will be cited for being out of ratio when LPA arrived.

During the course of the inspection, LPA conducted interviews with first hand witnesses (children and staff) regarding the incident. Staff indicated that child #1 ran into a shelf causing an injury that required medical. LPA also reviewed internal incident reports provided by the facility. LPA observed the area to appear in generally safe normal condition with no hazards. There was no corroborated disclosure indicating a Title 22 violation occurred regarding the injury. LPA observed child #1 at the childcare playing with no restrictions.

Staff performed first aid immediately and the facility also reported the incident in a timely manner (within 24 hours or the next business day). Based on interviews and review of internal reports there were no violations of Title 22 requirements.

The Notice of Site Visit (LIC 9213)must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. Exit interview was conducted with Licensee Sanabria.
