25 | Visit conducted in English and Spanish using Translation #14329 Licensing Program Analyst (LPA) Warren Birks conducted an unannounced Case Management inspection. LPA met with Licensee Leonor Sanabria who assisted LPA with verifying cleared deficiencies. This visit was also to provide LIC9224 acknowledgement forms March 8, 2022 inspection. Upon arrival LPA observed The Licensee with cleared assistant caring for 11 preschool children.
The following items have been cleared:
1. Capacity and Ratio: LPA observed the Licensee and assistant caring for 11 children. Therefore the facility is in currently in compliance. The previous capacity and ratio citation is cleared. Note: The Licensee indicated that she is using outside assistance when she has to run and errand in the afternoon.
There were no violations cited during this inspection. LPA provided LIC 9224 forms and this information for the previous visit: A copy of the March 8, 2022 report must be provided to the parent or guardian of every child and (including any newly enrolled children) for the next 12 months. The Acknowledgement of Receipt (LIC 9224 form must be maintained in each child’s file immediately upon receipt from parent/guardian). Licensee was provided with a copy of the Acknowledgement of Receipt of Licensing Reports (LIC 9224) form.
The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. This report along with a copy of the appeal rights was provided. Exit interview was conducted with Licensee Leonor Sanabria.
