An unannounced, required annual inspection was conducted on this date by Licensing Program Analyst (LPA) Bardo Baluyot. Upon arrival, LPA was greeted and guided on a tour of the pre-school classrooms by
Assistant Director, Alexis Godinez. Assistant Director guided LPA on a tour of the kitchen and of the pre school playground. Site Director, Heather Jenkins arrived a short time later.

Census: There were sixteen (16) children with 2 Staff in the 2's room, twelve (12) kids with 2 Staff in the 3's room, nineteen (19) kids with 2 Staff in the 4's Room and nineteen (19) kids with 2 Staff in the 5's room.

All areas identified on the Facility Sketch were inspected, including the food preparation area, storage and refrigeration; rest rooms, equipment, outside play area and overall conditions of facility. Furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting and ventilation were evaluated. Storage for children's belongings and an isolation area with a sink and toilet was inspected. Availability of drinking water was reviewed. Age-appropriate sinks and toilets were inspected for availability, good repair, water temperatures, toilet paper, towels, area safety and sanitation. First Aid supplies were inventoried for quantity and expiration dates; there is one First Aid kit in the classroom and one in the Director's office. There are no children currently on prescribed or over-the-counter medication.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. An Emergency Disaster Drill log is kept; the last fire drill was run 06/4/2021 at 10:45 A.M.

Food Service: Snack, lunch menus, food and snacks were reviewed for availability, quantity and appropriateness to children in care. Center provides lunch at 3:00 and P.M. snack at 5:00. Center provides all food. The kitchen (food preparation area) was toured for safety, cleanliness and proper equipment. The refrigerator in the main kitchen was also measured at 40 degrees. A review of cleaning and food supply storage areas was made. Uncontaminated drinking water was readily available. The kitchen, food preparation and storage areas appeared to be clean. The storage containers had tight-fitting covers.

Playground: Outdoor equipment was inspected for safety, cushioning material, good repair and age appropriateness. Required shade, drinking water and fencing were inspected. Play area was inspected for hazards and inaccessibility to bodies of water. All playground equipment appeared to be in safe condition. The surface of the outdoor activity space appeared to be maintained in safe condition and free of hazards.
The areas around and under playground equipment are cushioned with wood chips and/or artificial turf to absorb a fall. All materials and surfaces appeared to be toxic free.

Teacher-child ratios were observed and staff names recorded. Care and supervision was evaluated to determine if the basic needs of children are met appropriately. Sign in and out sheets and procedures were reviewed with staff, as was the policy of checking children for illnesses. Facility uses and electronic system. Personal Rights of children were discussed and observed by LPA. Inspection of required postings was made. There are eighty-eight (88) children currently enrolled. There are three permanent staff who are assigned to the room, two of whom also transport the children. There are three total staff who transport the children. The files of the five staff (including the ones who transport) and five children were reviewed for completeness including, but not limited to, Criminal Record Clearances for adults, Director Qualifications and verification of CPR/First Aid and health preventive practices documentation. There is a carbon monoxide detector located on the wall in the staff lounge; it was tested and is operable. here were no bodies of water observed on the premises (e.g. pools, spas, hot tubs, fish ponds, etc.) Per Licensee, there are no firearms or weapons stored at the Center. Storage of disinfectants, cleaning solutions, poisons and other items was observed in each classroom and in the kitchen.

The floors appeared to be clean and safe. There were no insects or rodents observed in the facility. Inspection authority was granted. Menus were posted. The indoor and outdoor activity space for the school-age children is physically separate from the other components.

The following is being cited in accordance to Title 22 of the California Code of Regulations. Please refer to 809D for documentation of deficiencies.

Upon receipt, the Licensee shall post the Notice of Site Visit. The Notice of Site Visit shall be posted for thirty (30) consecutive days. Failure to maintain posting as required will result in a $100 civil penalty.
