Licensing Program Analyst (LPA) Justin Dorsey contacted the facility via telephone to deliver complaint investigation findings due to COVID-19 and pre-cautionary measures. LPA Dorsey identified himself and discussed the purpose of the call. LPA Dorsey delivered the investigation findings to acting director Heather Jenkins.

As part of the investigation, LPA obtained a work order from a visit to the center by Southern California Gas Company dated 06/24/20 and the facilities staff and children's roster.

LPA Dorsey conducted interviews with staff members, facilities acting director, complainant, children, and parents of the program. On 06/26/20 LPA Dorsey received a complaint which alleged that the appliances are not kept in good repair. According to an interview with the complainant, the facility often smelled like gas. Per complainant, there was also an incident where the facility's dryer caught on fire and had to be put out by another staff member. LPA interviewed Staff #3, who the complainant claimed to put the fire out. Per Staff #3, this incident did not occur and there was no fire in the facility. LPA also obtained statements from additional staff who denied that a fire occurred at the center. LPA Dorsey also interviewed staff to gather information on the smell of gas in the facility. Per Staff #2 there were cases where teachers complained about the smell of gas however, the staff member was unable to recall when those concerns were brought to up. LPA also interviewed Staff #1, per Staff #1 there was a smell of gas in the facility. According to the interview with Staff #1, the facility only had a smell of gas for a “half an hour or less”. Staff#1 further stated that the gas company came and serviced the dryer which was venting from the inside as soon as the smell of gas was noticed. During the investigation, LPA Dorsey received the work order that was given to the facility by Southern California Gas Company dated 06/24/30.

Based on documentation gathered and conflicting statements presented above, the allegation has been determined to be unsubstantiated. A finding of Unsubstantiated means that although the allegation may have happened or are valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur.

An exit phone interview was conducted with acting director Heather Jenkins and a copy of this report was signed by LPA Dorsey. This report along with appeal rights will be sent via email to Acting Director, Heather Jenkins, who agrees to sign and date the report. This report was sent via email and an electronic read receipt confirms receiving the report. The facility representative was provided with the mailing address for the Monterey Park Regional office and agrees to send the original report by mail.
