On 06/30/2023 at 2:30 PM, Licensing Program Analyst (LPA) Katrina Chicote conducted an Unnannounced Case Management - Incident inspection to the above facility for the purpose of following up on an incident reported to The Department on 05/26/2023. LPA announced purpose of visit to Tina Coronel, Director, who allowed entry to facility. Census was taken.

On 05/26/2023, an incident was reported to The Department via fax in regards to construction occurring at the facility to repair the roof. Construction was set to begin on 05/26/2023 and continue through the week. The facility reported this incident to the Department within the required 24 hours and written report was submitted within seven days.

During inspection, staff were interviewed in regards to incident. Interviews with staff state that construction concluded the week after and that parents were informed both in person and on various platforms, including a classroom app ClassDojo. LPA observed app ClassDojo indicating parents were notified of construction. Director states classrooms did have to be closed to accommodate for construction, two classrooms at a time, and parents were provided a discount to accommodate for sudden closures of classrooms. Director states roofs were repaired due to time frame and not due to a noticeable issue to facility's physical plant. Director states they only observed water sports on ceilings but no leaking onto facility.

Based on the information obtained, LPA determined no deficiencies cited today.

A notice of site visit was given and must remain posted for 30 days.

Exit interview was conducted and report was reviewed with the Facility Representative, Tina Coronel, Director.
