Licensing Program Analysts(LPAs) Rita Ramos and Alicia Mooberry conducted an unannounced required/annual inspection. LPAs met with Ruth Cabral, Head Teacher, who guided analysts on a tour of the facility. This is a preschool program which consists of 2 classrooms; Room K1 and Room K2. Room K1 operates 2 sessions. The AM session is from 7:45AM to 11:15AM and the PM session operates from 12:15PM to 3:45PM. Room K2 operates a full day program from 7:45AM to 2:45PM. The facility operates from Monday through Friday. LPAs were later met by Natasha Jackson, Early Learning Center Manager.

All areas identified on this report were inspected. Upon arrival, the following staff were present during this inspection: Room K1: Staff #1, #2, #3 and #4 with 17 preschoolers; Room K2: Staff #5 and #6 with 18 preschoolers. The following was observed during the tour of facility:

Furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting and ventilation were evaluated. Children have their own cubby to store their belongings. Preschoolers in Room K1 do not nap at this facility, however, preschoolers in Room K2 do and their linens are washed weekly by a laundry service provided by the Long Beach Unified School District. Napping equipment (cots) for Room K2 were observed in a separate storage closet. Per Early Learning Center Manager, the isolation area is located in the Nurse's office if an ill child were to require it. Age appropriate sinks and toilets were inspected for availability and good repair in all restrooms. LPAs observed that facility does have a carbon monoxide detector. General sanitation was observed. Availability of indoor drinking water was observed in classrooms.


Disinfectants, cleaning solutions, medication and other items that are dangerous to children, were inaccessible to children. Per Head Teacher, there are no poisons stored at the facility.

This facility does not have a kitchen area. All food and meals are brought by a food delivery service provided by the school district and are stored in thermal heating containers or cooling containers. -----Page 1 of 3

Trash cans used to discard food have tight fitting lids.

Outdoor playground equipment is in safe condition, free of sharp, loose or pointed parts. LPAs advised
the Head Teacher that the children need to be within the direct care and supervision, including visual supervision, of the teacher(s) and supervising staff at all times. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All areas around or under high climbing equipment, slides, and similar equipment are cushioned with material that absorbs a fall. There is adequate shade in the play yard. Availability of outdoor drinking water was observed.

All individuals present have obtained a criminal record clearance or criminal record exemption as a condition of employment with the Long Beach Unified School District. There is at least one person trained in CPR and Pediatric First Aid present during this inspection. The LIC 308 Designation of Facility Responsibility was observed to be posted.

Children’s Records were reviewed for completeness; Inspection of required forms was made. LPAs issued the Children’s Record Review (LIC 857) to the Early Learning Center Manager during this inspection. In review of children’s records, files contain information including, but not limited to the following: Name, address and telephone number of the child's authorized representative and of relatives or others who can assume responsibility for the child if the authorized representative cannot be reached when necessary.

LPAs also issued the Review of Staff records (LIC 859) to the Early Learning Center Manager during this inspection. The LIC 859 documents the staff files that were reviewed during this inspection.

Children's roster was reviewed and is current. Sign-in and Sign-out sheets were reviewed. Children present were signed-in. Disaster drill log was available, last drill was conducted on 06/11/19.

Menus are posted one month in advance where it is visible by the child's authorized representative. Menus for the past 30 days are available upon request. Preschoolers in the AM session in Room K1 are provided with a breakfast and preschoolers in the PM session are provided with a lunch. Preschoolers in Room K2 are provided with a breakfast, lunch, and snack. Snacks were reviewed for availability, quantity and appropriateness to children in care.-------------------Page 2 of 3

First Aid supplies were observed in the classroom in a kit-box. According to the Head Teacher, medication is only administered to a child when accompanied with a doctor's note. Medication was observed to be stored in an inaccessible area in Room K2.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. LPAs advised the Early Learning Center Manager to access forms, regulations and quarterly updates on the Child Care Licensing website at: www.ccld.ca.gov.

There were no deficiencies cited during today’s inspection.

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. Exit interview was conducted with Natasha Jackson, Early Learning Center Manager, including, but not limited to Provider Rights, Appeal Procedures and Agencies Consultative Role.
