Licensing Program Analysts (LPAs) Rita Ramos and Lili Babcock conducted an unannounced required 1 year inspection on 09/02/21 at 8:50AM. LPAs met with Ruth Cabral, Head Teacher, who guided analysts on a tour of the facility. This is a preschool program which consists of 2 classrooms; Room K1 and Room K2. Facility operation hours are Monday through Friday from 7:45AM to 2:45 PM in Room K1 and Room K2 operates 2 sessions. The AM session operates from 7:45AM to 11:15AM and the PM session operates from 12:15PM to 3:45PM. LPAs were later met by Jenny Acosta, Administrator of Support Services. This facility is located on the premises of Buffum Elementary School in the city of Long Beach.

All areas identified on this report were inspected. Upon arrival, the following staff were present during this inspection: Room K1: Staff #1 and #2 with 10 preschoolers; Room K2: Staff #3 and #4 with 8 preschoolers. The facility was observed to be within the license capacity and limitations. The following was observed during the tour of the facility:

Furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting and ventilation were evaluated. Children have their own cubby to store their belongings. Linens are washed weekly by a laundry service for children in Room K1. Children in Room K2 do not nap since they are part time. Napping equipment (cots) were observed in a separate storage closet in Room K1. LPAs advised that staff ensure that blankets are not touching one another when placed on top of the cots because the cots are being stored stacked on top of each other. Per Head Teacher, the isolation area is located in the nurse's office. Age appropriate sinks and toilets were inspected for availability and good repair in all restrooms. General sanitation was observed. Availability of indoor drinking water was observed in classrooms via disposable cups and a water jug. The classrooms also have functioning drinking faucets, however, due to COVID-19 the facility is providing disposable cups and a water jug.

When inspecting Room K1, LPAs observed that the latches on the cabinet under the sink was easy to detach. LPAs were able to open the cabinet and observed disinfectants, cleaning solutions, and other items that are dangerous to children. LPAs advised that the facility place a more secure latch to ensure that children are not accessing the disinfectants and cleaning solutions. The Head Teacher was able to place a more secure latch during the inspection. The Head Teacher also states that there are no poisons stored at the facility.

All kitchen areas/food preparation areas and food storage areas are kept clean and are free of litter, rubbish, rodents, and/or any other vermin. All storage containers for solid waste, including moveable bins have tight-fitting covers that are kept on, and in good repair. Trash cans used to discard food have tight fitting lids.

Outdoor playground equipment is in a safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition. LPAs advised the Head Teacher to remove or wind up the water hose that was observed unraveled to prevent a tripping hazard. LPAs did observe a crack on the corner edge of the concrete/stucco wall and advised the Head Teacher and Administrator to have it checked and repaired. All areas around or under high climbing equipment, slides, and similar equipment are cushioned with material that absorbs a fall. There is adequate shade in the play yard. Availability of outdoor drinking water was observed. LPAs advised that no children shall be left without the supervision of a teacher at any time.

All floors were observed to be clean and safe. All materials accessible to children were observed to be toxic-free There are no firearms stored on the premises. There are no pools or bodies of water at the facility.

All individuals present have obtained a criminal record clearance or criminal record exemption as a condition of employment with the Long Beach Unified School District. There is at least one person trained in CPR and Pediatric First Aid present during this inspection.

Children’s Records were reviewed. In review of children's records, LPAs observed that the facility has electronic files that were printed for LPAs during the inspection. LPAs observed that forms that require a signature are missing signatures and the facility is typing "via phone." The Licensee recently submitted a request to use electronic forms, however, it is still pending review and approval -----Page 2 of 4

The name, address, and telephone number of child’s authorized representative are on file and also a medical assessment.

LPAs also reviewed staff records. The review of Staff records was documented on the LIC 859. Staff present did have proof of the AB 1207 Mandated Reporter Training certificate on file. Staff present did have proof against TB, measles, pertussis, and influenza. All staff have been given on the-job training sanitation principles, housekeeping, including universal health precautions. LPAs also issued the Review of Staff records (LIC 859) to the licensee during this inspection. The LIC 857 and the LIC 859 documents the staff and children’s files that were reviewed during this inspection.

Children's roster was reviewed and is current. Sign-In and Sign-Out sheets were reviewed. Children present were signed in.

Menus are posted one month in advance where it is visible by the child's authorized representative. Menus for the past 30 days are available upon request. Snacks were reviewed for availability, quantity and appropriateness to children in care. Children in Room K1 are provided with a breakfast, lunch, and snack due to full-day enrollment. Children in Room K2 from the AM session are provided with a full breakfast and children from the PM session are provided with a full lunch.

First Aid supplies were observed in the classroom in a cabinet. According to the Head Teacher, medication is only administered to a child when accompanied with a doctor's note and is stored in each classroom where it is inaccessible to children in care.

This facility provides Incidental Medical Services – IMS. LPAs reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. LPAs advised the Head Teacher to access forms, regulations and quarterly updates on the Child Care Licensing website at: www.ccld.ca.gov. LPAs provided the Head Teacher with a copy of the Effects of Lead Exposure form during the inspection and discussed the following Provider Information Notices (PINs) Required Lead Testing: PIN 20-01 CCP Effects of Lead Exposure, and Guardian User Account Access - PIN 20-20-CCLD.

There were no deficiencies cited during today's inspection.

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Exit interview was conducted with Ruth Cabral, Head Teacher, including, but not limited to Provider Rights, Appeal Procedures and Agencies Consultative Role.
