25 | Licensing Program Analyst (LPA) Alicia Mooberry conducted an unannounced annual random inspection on this date. Upon arrival, LPA met with Licensee, Donice Moore, who guided on a tour of the facility. LPA provided the entrance checklist LIC 126. Individuals residing in the home are discussed and noted. Per licensee, operating hours are Monday-Friday 6am-1am.
There were 3 children present. Per licensee, there are 8 children enrolled. LPA confirmed with licensee that all adults residing/working in the home have received criminal record clearance.
The facility is a one story, 3-Bed, 2-Bath home. Per Licensee the following areas are used for day-care: Living room (main day care room) and restroom in the hallway
Off limit areas include: All Bedrooms, 1 Restroom, detached garage and kitchen.
LPA observed safety gates in the kitchen and leading to the den making areas inaccessible to children in care
LPA observed required posted documentation in facility entrance which included: Facility License, Publication (PUB) 394- Notification of Parent Rights and Licensing Form (LIC) 9148- Earthquake Preparedness form. LPA observed completed facility records including; LIC 9040- Facility Roster, LIC 610- Facility Disaster Plan.
Smoke and carbon monoxide detectors were tested and are operable. Fire extinguisher indicated fully charged. Licensee was reminded that fire extinguisher needs to be serviced yearly. The home maintains telephone service via cell phone. The home is observed to be clean and orderly. There are toys and other age appropriate material available for children.
The bathroom that children use is located in the hallway and observed to be clean and free of hazards.
