On 10/28/2022 at 10:00 AM, Licensing Program Analyst (LPA) Carolyn Tuba conducted an unannounced required Annual inspection. LPA met with Director, Wendy Mitchell and a COVID-19 risk assessment was conducted prior to entering the facility. There have been 3 cases in the last 2 days and LPA switch to an N-95 mask and report was written off the premises. At 10:30 AM LPA was guided on a tour of the facility indoors and outdoors. This is a preschool age program licensed for 64 children, which operates Monday through Friday from 7:00 AM to 6:00 PM. Per the Director, there are 62 children enrolled. Director provided LPA with a current daily Schedule and Parent Handbook.

The following classrooms were inspected: Room 2 is being used as an isolation room. Room 4 had 17 children in care with 3 teachers in care. Room 5 had 8 children in care with 1 teacher and on the other side of Room 5 there was 6 children with 1 teacher in care and 1 volunteer. Room 3 is called a Cultural Classroom used for Extension of Learning. Room 1, which is not connected to the main facility and located adjacent from the outside play area had 12 children and 1 Teacher in care and the classroom has its own restrooms and staff bathroom is located there as well. Teacher-child ratios were observed to be in accordance with Title 22 Regulations. Staff names were recorded. All children were observed to be under supervision, including visual supervision. Licensed facility is within the conditions, limitations, and capacity specified on the license.

The classroom furniture and equipment were inspected for good repair, free of sharp, loose, or pointed parts. All indoor classrooms were inspected to ensure that the floors have a surface that is safe and clean. All toilets and washing facilities are in safe and sanitary operating conditions. All materials and surfaces accessible to children are toxic free. Availability of drinking water is provided by a water service and filtration water pitchers kept in each classroom. Children also have water bottles from home available to the children. LPA advised to make sure names are on each bottle. Room 1 currently has a wall heater, which is not in use. LPA advised to have the Director add a secured barrier or to have Gas Company come out and permanently shut it off and tag it. Director would provide LPA with a copy of the receipt.

The facility provides morning and afternoon snack and parents bring lunch from home. Food is kept in the kitchen area with expiration dates. A snack menu is located on the refrigerator and in the main lobby area for parents to view. LPA advised Director that it should be written out for the entire month.

Licensing staff observed that there are functioning smoke and carbon detectors in the facility. Licensing staff observed a fully charged and serviced fire extinguisher last serviced on 10/05/22. Emergency disaster drill was last conducted on 10/2022. There are First Aid kits in the kitchen area; the supplies were inventoried.

Outdoor playground equipment is in safe condition, free of sharp, loose, or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All areas around or under climbing equipment, slides, and similar equipment are cushioned with a soft foam material that absorbs falls, however there is some areas of the foam that is damaged from weather and day-to-day use, LPA took photos. LPA advised those areas need to be repaired and/or replaced. There is adequate shade in the play yard. There are tricycles stored in the cemented play area. There are 4 picnic tables under a shaded covered patio. Area has a secured gate all around the entire facility and a code is required to enter the facility, as well as a ringer.

Sign in and out sheets are kept near the entrance of the facility and were reviewed. There are 2 Guinea Pigs that are kept in cages by the front entrance. LPA advised to keep cages clean for the health and safety of children. Director reported that she cleans cages every Friday and that the pets have only been here for two weeks.

Licensing staff reviewed staff records and they were complete. Clearance for adults and verification of CPR/First Aid and health preventative practices documentation was reviewed. Children’s records were reviewed to ensure that Identification and Emergency form and a medical assessment are on file. Criminal Records SB792 (Immunization Requirements for Staff and Employees) were discussed with the Director. The Director and staff currently have immunization documentation.

AB1207 Mandated Child Abuse Reporting – On or before March 30, 2018 any person who works in a childcare facility shall complete the training and renew the training every 2 years. Website provided: https://www.mandatedreporterca.com/training/child-care-providers.


Per Director Medication is kept in a container in the kitchen. There are few children currently who have medication at the school and staff members are aware on how to administer in the event it is required. LPA advised to make sure that all medication has the child’s name with a prescription label and that it is not expired. An IMS plan is available in the child’s file. Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. LPA advised the licensee to access forms, regulations and quarterly updates on the Child Care Licensing website at: www.ccld.ca.gov.



Director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
