Licensing Program Analyst (LPA) Judy Mora conducted an unannounced Required 1 Year inspection. LPA met with designated facility Director, Shaquiesha Hampton. This is an infant program licensed for 24 children which operates Monday – Friday from 6:30 AM – 6:00 PM. Per the Director there are 24 children enrolled. There is a combination center with a preschool license on site, LIC # 198016995. The infant program is physically separate from other components both indoors and outdoors.

At approximately 12:00 PM LPA Mora began facility tour with Director. All areas identified on the facility sketch were inspected. LPA observed 06 infants with 02 staff in the lullaby room, 06 children with 01 staff in the Huggable Hills Room and 08 children with 02 staff in the Rainbow Room. Teacher-infant & toddler ratios were observed to be in accordance with Title 22 regulations. The Licensee is within the conditions, limitations, and capacity specified on the license. Staff names were recorded. All infants were observed to be under direct visual supervision of a staff person at all times.

LPA observed the facility to be clean, safe, sanitary and in good repair. Furniture and equipment was inspected for good repair, free of sharp, loose, or pointed parts. All indoor classrooms were inspected to ensure that the floors have a surface that is safe and clean. All toilets and hand washing facilities are in safe and sanitary operating conditions. All materials and surfaces accessible to children are toxic free. At this time, the Sick Bay Room is used as the isolation area.

Parents of children enrolled provide all milk and food items. There is drinking water available in all indoor classrooms and drinking containers / jugs are taken outdoors. All storage containers for solid waste, including moveable bins, have tight fitting covers on and are in good repair. Disinfectants, cleaning solutions, poisons and other items that are dangerous to children are stored in an area inaccessible to children. Storage areas for poisons are locked. Facility has one or more functioning carbon monoxide detectors that meet statutory requirements. The licensee takes measures to keep the facility free of flies, other insects and rodents.

Infant Care: LPA observed age appropriate furniture and equipment, but not limited to cribs, cots, changing tables, and feeding chairs. LPA observed and inspected sleeping equipment to ensure there is a sufficient amount. LPA observed that cribs and/or play yards are free from loose articles and objects. There are no objects hanging above or attached to the side of the crib. Pacifiers were observed to not be attached to anything and are labeled. Licensee was advised that infants shall not be swaddled while in care and all infants up to 12 months should be placed on their back for sleeping.

Title 22 Regulation Section 101429(a)(2)(b) Responsibility for Providing Care and Supervision for Infants was discussed with the Licensee, including but not limited to documentation that shall be maintained. LIC 9227 (Individual Sleeping Plan) for infants up to 12 months was explained and issued to the Licensee.

Outdoor play equipment was observed to be in good condition, free of sharp, loose or pointed parts. Outdoor activity space surface is maintained in a safe condition as is free of hazards. The Director states that there are no bodies of water on the premises and LPA did not observe any bodies of water during this visit. Areas around and/or under climbing equipment, swings and slides have sand and grass cushioning material to absorb a fall; climbing apparatuses are single level only. Director states there are no weapons or firearms on the premises.

Sign in and out sheets were reviewed to ensure that the person who signs the child in and out uses their full legal signature and records the time of the day. Staff Records were reviewed to ensure that a health screening report is on file and that Infant staff are qualified. Children’s Records were reviewed to ensure that Identification and Emergency form and a medical assessment are on file. Criminal Records Clearance for adults and verification of CPR/First Aid and health preventative practices documentation was reviewed.



SB792 (Immunization Requirements for Staff and Employees) was discussed.

AB1207 Mandated Child Abuse Reporting – On or before March 30, 2018 any person who works in a child care facility shall complete the training and renew the training every 2 years. Website provided: https://www.mandatedreporterca.com/training/child-care-providers.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. An annual continuation inspection will be conducted in order to further review infants files for required documents and staff files for required qualifications.

The following was discussed:

· Reporting Requirements.


· Individuals who are 18 years of age or older living in the home must obtain a criminal record clearance. Individuals within one month of their 18th birthday must be fingerprinted immediately. Failure to obtain a criminal record background check clearances prior to initial presence in the home will result in an immediate $100.00 dollar or more per day Civil Penalty.
· The facility shall ensure that each child is accorded the following personal rights: to be accorded safe, healthful and comfortable accommodations.
· All personnel shall be in good health and shall be physically and mentally capable of performing assigned tasks; personnel that pose a threat to the health and safety of children shall be relieved of their duties.
· All personnel shall be given on the job training in housekeeping, sanitation principles and universal health precautions.
LPA advised the licensee to access forms, regulations and quarterly updates on the Child Care Licensing website at: www.ccld.ca.gov. At this time, the licensee is in compliance with California Title 22 Regulations. Therefore, there are no citations being issued today.
The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
Exit interview was conducted with Director, Shaquiesha Hampton. Appeal Rights and Procedures explained and provided.
