25 | ANNUAL INSPECTION CONDUCTED IN ARMENIAN Licensing Program Analyst (LPA) Anomeh Eivazian conducted an unannounced annual inspection to the above facility on 07/14/2022. LPA arrived at the facility at 12:19 PM and met with Karine Sahakyan, Licensee who guided analyst on a tour of the facility. Also present during this inspection, were Mariam Khachatrian and Carmen Hovhannisyan, Licensee’s Assistants. Per licensee, facility hours are Monday to Friday from 7:00 a.m. to 10:00 p.m. LPA provided a copy of Entrance Checklist—Family Child Care Home (LIC126) to licensee.
This is a one story home which consists of 3 bedrooms, 2 bathrooms, kitchen, dining room, living room, detached converted garage to a play room in the back yard, bathroom in the detached converted garage, laundry room in the backyard, front yard and backyard (fenced). The children use the 2 bathrooms in the hallway, living room, dining room, 2 bedrooms, detached converted garage to playroom in the backyard for limited activity only, bathroom in the detached converted garage in the backyard, and backyard (fenced). Per licensee, areas off limits to children and parents include: Licensee's bedroom, laundry room in the backyard, and front yard. During this inspection individuals who reside in the home were discussed and notes on Confidential Name List (LIC811) and attached to this report.
All areas identified on the facility sketch that are accessible for children to use were inspected for safety, comfort, and cleanliness. At 12:19 pm., LPA arrived to the facility through the side gate which leads to the backyard. LPA observed 14 children (one being 6 years old and one being school age) were having lunch outside in the backyard. Next LPA toured the detached converted garage to a playroom and bathroom in the converted garage and observed children toys, tables and chairs in the detached converted garage. Per licensee, children use the detached converted garage to a playroom for limited activity only, about two hours in the mornings and two hours in the afternoons. Per licensee, children either eat outside or inside the home and sleep inside the main home.
